Description
This is a combined synopsis/solicitation for commercial items. Non-Personal services for Television/Satellite Operation and Maintenance at Wilford Hall Medical Center Complex, Bldg. 4550, 2200 Berquist Ave. on Lackland AFB, TX. in accordance with the attached Performance Work Statement (PWS). This announcement constitutes the only solicitation. Offers are being requested and a written solicitation will not be issued. Solicitation FA3047-05-T-0011 is issued as a request for quote (RFQ). The Basic Performance period is 01 Oct 05 – 30 Sep 06, with four (4) option periods (1 Oct 06 – 30 Sep 10). A Firm-Fixed Price contract is contemplated. This solicitation document, incorporated provisions and clauses are those in affect through the Federal Acquisition Circulars 05-01 thru 05-04 and Defense Acquisition Circular 91-13 (correction). North American Industry Classification System (NAICS) code is 811211, Consumer Electronics Repair and Maintenance; Standard Industrial Classification (SIC) is 7622, Radio and Television Repair. Size Standard of $6,000,000 applies to this procurement. This solicitation is a 100% set-aside for small business. Prospective contractors must be registered in the Central Contractor Registration (CCR) at www.ccr.gov and Online Representations and Certifications Application (ORCA) at http://orca.bpn.gov . YOU WILL NEED CURRENT VEHICLE REGISTRATION, DRIVERS LICENSE, AND INSURANCE IN ORDER TO OBTAIN A PASS. rform services and support compatible with the medical facility’s operating capacity and equipment. 2. SERVICE DELIVERY SUMMARY (SDS) P.0. # Performance Objective PWS Paragraph Performance Threshold 1. Contract personnel shall respond within three duty hours service calls. 2.1.1 2 late responses per month 2. Equipment taken off site for repair shall be repaired and returned within 7 working days. 2.1.2 1 late return per month 3. Total repairs per each piece of equipment shall not exceed $50 for parts or 2 hours for labor without authorization by government representative. 2.1.4 100% authorization by government representative 2.1. Description of Duties: Contract personnel shall maintain approximately 300 televisions (from 5” to 43”, Analog and Plasma) of various brands and age in working condition. Provide all services required to ensure the government television systems in WHMC are operational to include, but not limited to, installing, relocating, testing, calibrating, and replacing related equipment. Equipment includes, but is not limited to; Satellite Dish and UHF-VHF Master Antennas, televisions, DVD/VCR, Head-End equipment, support shelves/brackets, swing arm, camera, monitor, amplifier, switcher, splitter, cabling, and jacks. 2.1.1. Service Calls: Contract personnel shall call-in within 1 hour to acknowledge receipt of service request, and report within three hours of being called to the government representative at the designated location in MTF, Lackland AFB. They shall receive instruction on the requirements of the call and be issued keys, as necessary, to provide access to service panels and roof for antenna service/calibration. Contract personnel shall perform on-site inspection & testing of government satellite television system equipment; system adjustment, repair or replacement of components. Contract personnel shall also out-brief the government representative after completion of the service, and turn in keys. 2.1.2. Off-Site Repairs: Maintenance and Repair shall be accomplished on-site during regular duty hours when possible. If required to remove equipment to contractor’s facility for repair, written authorization by the government representative shall be obtained. Turn-around time for equipment removed from the MTF shall not exceed 7 calendar days, without authorization from the government representative. Contractor shall not exceed 2 labor hours and/or $50.00 parts per each piece of equipment removed from the MTF without authorization from the government representative. 2.1.3. Install, Replace, or Relocate: Equipment shall be installed, replaced, and/or relocated as required. Contract personnel shall document the manufacturer, identity numbers, and room location of the equipment on the Service Call Form / Trouble Call Form. Cable routes to outlets shall not be exposed, but must be inside walls or in wire molding. Above ceiling routes shall be in cable racks, ties, or conduit. Swing Arm Supports shall be left in the open position when television set is not attached, due to safety requirements. 2.1.4. Reimbursable Maximum Parts Cost: The total cost to repair equipment shall not exceed the reasonable replacement cost, as determined by the government representative. Contractor shall make recommendations based on condition/status of equipment, and a determination will be made by the government representative pertaining to repair or replacement. For minor repairs (two hours labor) /parts (below $50 per each piece of equipment), contractor is authorized to proceed without authorization from government representative. Reimbursement will be for the cost of materials and parts required to maintain the systems when government replacement items are not available. All cost exceeding set limits must be approved in writing by designated government representative. 2.1.5. Satellite System: Contract personnel shall tune in satellite television coordinates (Analog and Digi NOTE: THIS NOTICE MAY HAVE POSTED ON WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (15-AUG-2005).
