- Request Service Maintenance Agreement (SMA Plus coverage)on the ADC/IMPAX digital imaging network and picture archive and capture system (DIN-PACS). In accordance with the Statement Of Work and shall be attached to this announcement. Period of Performance 09/25/05 - 09/24/06 one year coverage. This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being required and a written solicitation will not be issued. The Performance Work Statement (SOW) and other requirements will be attached/posted. The following clauses are applicable to subject solicitation: FAR 52.212-1 (Instructions to Offerors), 52.212-2 (Evaluation - Commercial Items), 52.212-3 (Offeror Representations and Certifications - Commercial Items), 52.212-4 (Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items). IAW clause 52.212-2, the evaluation factors shall be: Price and Price related factors, Conformance to performance specifications (SOW) and past performance. This solicitation incorporates provisions and clauses that are in effect through Federal Acquisition Circular (FAC)97-26. NAICS Code 811212. Place of performance/FOB Robins AFB, GA. STATEMENT OF WORK SMA Plus Service Maintenance Agreement for ADC/IMPAX (AGFA) (Equipment Service Maintenance Agreement) SECTION TITLE PAGE -1- DESCRIPTION OF SERVICES 2 -2- SERVICE DELIVERY SUMMARY 3 -3- GOVERNMENT FURNISHED PROPERTY AND SERVICES 3 -4- GENERAL INFORMATION 3 OPR: 78 MDSS/SGSAR SEP 2005 (DSN 497-8184) Statement of Work (SOW) SMA Plus Service Maintenance Agreement for ADC/IMPAX (AGFA) 1. Description of Services for: 78 MDSS/SGSAR Robins AFB, GA 1.1. The contractor shall provide hardware and software maintenance services for all Picture Archive and Capture System (PACS) components itemized herein. These services will include the following components/software programs: 1.1.1. ADC System hardware components (ID Station, Compact Digitizer, PC + Monitor (2), Processor SE, and Solo Digitizer) 1.1.2. Accessory hardware components (IMPAX Vidar Pro Plus Digitizer, Dell Optiplex GX260, Drystar 2000 printer, and Drystar 3000 printer) 1.1.3. IMPAX System hardware components (OS3000, 258 Platter, 4.3 GB PCI I, DS3000 (2), Network Gateway, QC Workstation, RIS Gateway, RS5000, and Web1000 Base) 1.1.4. PACS software components (DS3000, Relay, Service Tools, Web1000, Web1000 Service Tools, ADCC Processing, ADC2 Identification System (PRID), and IMPAX TS5) 1.2. Maintenance services will include: 1.2.1. Installation/configuration of servers, workstations, printers, and associated ADC/IMPAX components included in service maintenance agreement 1.2.2. Preventive maintenance, troubleshooting, image quality audits, and repair (to include replacement parts and delivery) 1.2.3. Coordination with MTF/base infrastructure personnel to assist with isolation/correction of problems that affect ADC/IMPAX connections/components or AGFA software 1.2.4. All future IMPAX and Talk Technology software releases (i.e., version 3.4 to 4.0) and applicable software enhancements/updates (i.e., version 4.1 to 4.5) 1.3. All services will be provided either onsite, via telephone, or via dial-in remote access. Labor charges, travel charges, and all expenses will be included for all on-site services between the hours of 0800-2000, M-F, excluding federal holidays. Telephone and dial-in remote access support will be available through the contractor help desk, 24 hours per day, 7 days per week. 1.4. The contractor will be responsible for notifying the MTF representative of any discrepancies noted during preventive maintenance checks or image quality audits. The contractor shall also be responsible for notifying the MTF of available hardware or software upgrade programs, regardless of whether installation of the specific item is covered hereunder. Additionally, the contractor will notify the MTF of any professional services (such as Applications training, workflow implementation, project management, or data migration) that could be useful to improve MTF services. 2. Service Delivery Summary (SDS) Performance Objective SOW Reference Performance Threshold Install replacement parts. Para. 1.2.1. and 1.2.2. Install and configure within 5 business days of help desk request. Provide technical phone support and dial-in remote access. Para. 1.3. Provide phone or dial-in support, or elevation of issue to on-site support, within 2 phone calls to help desk. Provide on-site support. Para. 1.3. Respond within 4 hrs of notification. 3. Government Furnished Property and Services 3.1. The government will provide a copier and fax for official use only. 3.2. The government will provide station, local and long distance telephone services for official use only. The use of government telephones by the Contractor shall be permitted in direct support of the contract. 4. General Information 4.1. The contractor shall understand and adhere to the Privacy Act of 1974. The Contractor may be required to have access to live data during the performance of this task order. Any information, records, or data the Contractor may have access to may be of a sensitive nature. The Contractor shall not divulge any information about Government files, source selection activities and processes, or any other sensitive information to anyone not authorized access to such information. 4.2. All covered hardware/software will be located at the 78th Medical Group/SGSAR office or SGSI computer room within building 700 on Robins AFB, GA, or within the outlying buildings of the 78th Medical Group. All work will be performed on RAFB or via dial-in remote access. 4.3. Occasional emergency workload peaks, or changing government priorities, may necessitate that the contractor provide services after-hours. All after-hours services must be approved in advance by the MTF representative or contracting officer. 4.4. Performance of Service During Crisis 4.4.1. DoDI 3020.37, Continuation of DoD Essential Services During Crisis, applies. 4.4.2. Services deemed to be mission-essential include all components of the DIN-PACS with the exception of film printing hardware and software. All other covered components are critical to continued accomplishment of the mission of the MTF.
