- GENERAL SERVICES ADMINISTRATION CENTRAL HEATING AND REFRIGERATION PLANT Concrete Slab Replacement Over Five Transformer Vault Rooms THERE WILL BE A PREBID CONFERENCE ON SEPTEMBER 26, 2005 10:AM LOCATION: CENTRAL HEATING PLANT 13TH & C, STREETS, S.W. WASHINGTON DC 20407 BIDS ARE DUE ON OCTOBER 14, 2005 AT 12:00 NOON ADDRESSED TO MR. JAMES M. JOHNSON JR. ANY BID RECEIVED AFTER THAT TIME WILL BE CONSIDERED NON RESPONSIVE. A. Scope of Work 1. The contractor shall furnish all of the necessary equipment, labor, supplies, and supervision for replacement of removable concrete slabs and concert walkway over all five high voltage transformer vault rooms. 2. The contractor shall place a temporary work fence around the perimeter of the vault and sidewalk areas being replaced. The temporary fencing shall be placed in such a manner that will allow access to and from at least one (1) emergency exit at all times and the roll up door on refrigeration side of the CHP. 3. The contractor shall install temporary protection shields above all transformer?s doing contraction using plywood to be erected over existing transformer units in vaults being repaired. This protection should be in placed in such a manner that it will protect the transformers from falling debris during repair of concrete beams, columns, and sidewalk. 4. The contractor shall where applicable individual existing steel railing units shall be removed. The remaining steel railing that can not be completely removed will be treated in the following manner. Chip out and remove concrete around the anchor bolts. Removed existing anchor bolts and provide temporary support for sections while concrete sidewalk is being replaced. 5. The contractor shall precast concrete removable panels forming the roof of the vaults (30 panels measuring 6 feet long x 28 inches wide x 6 inches thick) the old slabs shall be removed and disposed of. New precast concrete removable panels shall be constructed with new steel angle iron and new left out bolt hard wear. 6. The contractor shall remove and replace both concrete drive ways on east side of the CHP with new concrete with exposed aggregated stone finish. The two drive ways are approximate area 30ft x 10ft. 7. The contractor shall construct new panels off site with new steel angel frames 6 feet long x 28 inches wide x 6 inches thick coated with coat FX-509 primer and two coats of FX-509 rust inhibitive coating. Number 4 reinforcing bars shall be placed in the new angel form work to match the location of the original panels. The new concert shall be placed to the proper thickness and the surface shall be an exposed aggregated stone finish. Panels should cure for a minimum of 7 Days 8. The contractor shall repair all spalling concert columns and beams inside the 5 High voltage transformer rooms. Approximately 500 square feet of spalling concert beams and approximately 10 cubic feet of spalling concert columns 9. The contract shall chip out and remove deteriorated concert. Clean all exposed reinforcing steel prime all reinforcing steel. Place temporary forms work around columns and beams in such a manner that will return these areas back to their original sizes and configuration. 10. Security Clearance Requirements: (Non-Classified Contract) a. Unless otherwise specified, the contractor will submit to the COR at least fifteen work days before the starting date of the contract, on two completed Form FD-258, "Fingerprinting Chart," and one completed GSA Form 176T "Statement of Personal History," for the Contractor and all employee who have access to the building in performance of the contract work. b. These forms will be submitted for replacement employees before entrance on duty. The government will furnish necessary forms. If the CO receives an unsuitable report on any employee after processing of these forms, or if the COR finds a prospective employee to be unsuitable or unfit for his assigned duties, the contractor shall be advised immediately that such employee cannot continue to work or be assigned to work under the contract. c. For employees cleared through this process while employed by a contractor who is subsequently replaced by another contractor in the same building, the new contractor shall only be required to submit another set of these forms if the employee has not been cleared within the last three years, or if required by the COR. All contract employees are required to be cleared every three years. d. The government shall have and exercise full and complete control over granting, denying, withholding or terminating clearances for employees. The government may, as it deems appropriate, authorize and grant temporary clearance to employees of the contractor, However, the granting of a temporary clearance to any such employee shall not be considered as assurance that full clearance will follow as a result or condition thereof, and the granting of either temporary or full clearance shall in no way prevent, preclude or bar the withdrawal or termination of any such clearance by the government. B. General Contract Specifications 1. The contractor shall complete the work of the contract within 120 calendar days from the date of the notice to proceed. Failure to complete the contract within the stipulated time shall result in liquidated damages of $100 being assessed for each day of the contract in excess of the 90 days. 2. The work required under this contract is to be performed at the Central Heating Plant as certain and comply with the prevailing safety and security regulations at Central Heating Plant. 3. The contractor shall comply with all GSA Plant and OSHA safety and security regulations and shall ensure that his employees are properly supplied with the appropriate identification and safety equipment in accordance with the requirements stipulated at the Central Heating Plant. The contractor shall inform the Contracting Officer of work he intends to perform that could affect the safety of personnel or interfere with plant operations. 4. The work required under this contract may be performed between the hours of 7:00 am till 4:00 pm. Monday thru Friday and all day Saturday and Sunday 7:00 am. Until work is finished to minimize interference and disruption of plant operation during normal working hours. 5. The contractor shall clean up the work site at the end of each workday and remove all work-related debris or litter upon completion of the project. 6. The contractor is responsible for the removal of his waste and shall not use Government-owned or controlled equipment to contain, store, or otherwise handle his trash or debris. The contractor shall provide sufficient containers to remove all waste created by his operations and remove it from Government property as soon as practicable after completion of his work. The contractor is responsible for assuring that all waste from Government work is disposed of only through a facility that has been certified by the appropriate State or local agency for waste management, or by the Environmental Protection Agency as applicable. 7. Upon the conclusion of all work, a comprehensive inspection shall be performed by the Government representatives prior to the issuance of final approval and acceptance by the Government. 8. It shall be the responsibility of the contractor prior to bidding this job, to seek clarification to any unclear items on the specification and inform themselves of existing site conditions. Failure to do so will in no way relieve the successful contractor from work that may be required to carry out the terms of the contract, in accordance with the true intent and meaning of the specifications at no additional cost to the Government. C. Contractor Qualifications At the discretion of the contracting officer, the successful contractor may be required to submit a list of three references to attest to their experience and previous successful completion of jobs of the type and magnitude fied under this solicitation. The field service technician must have at least 5 years experience in performing work of this nature. D. Basis of Offer The contractor shall quote a firm, fixed unit price to perform the work as specified under the terms and conditions of the contract.
