- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation RFQ 506-00469-06 is being issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-5 (8/26/2005). NAICS Code 811219, sized standard $6 Million applies. STATEMENT OF WORK: 1. SCOPE OF WORK: The Contractor shall provide all necessary parts, labor and materials as necessary to perform four (4) Preventive Maintenance (PM) inspection(s) per year and unlimited emergency repair services on the stated Government owned equipment, for the period of October 1, 2005 through September 30, 2006, in strict accordance with all schedules, specifications, terms, conditions and provisions of this contract. 2. EQUIPMENT: a) AMSCO ? 6000 Tunnel Cage Washer, S/N 7459, located in Bldg. 31; b) AMSCO ? 4200 Cage and Rack Washer, S/N 7450, located in Bldg. 31; c) AMSCO ? 470 Laboratory Glass Washer, S/N 3627794010, located in Bldg. 31, Rm. 105; d) AMSCO ? Hop Lab Washer, Washer 1021, S/N 3631487071, VA# 6640-5740, located in Bldg. 22, Rm. G-14; e) AMSCO Eagle 2000 Series Sterilizer with cart 2041, S/N 0128987-001, located in Bldg. 22, Rm. G-14; f) GETINGE/CATSTLE ? 3633 Manual Sterilizer, S/N 94M56451, located in Bldg. 31, Rm. 29, Basement; g) GETINGE/CATSTLE ? 3633 Manual Sterilizer, S/N 94156381, located in Bldg. 31, 1st floor, Rm. 2; h) GETINGE/CATSTLE ? 3833 Manual Sterilizer, S/N 94M56456, located in Bldg. 31, Rm. 105; I) GETINGE/CATSTLE ? 3622 Sterilizer, S/N 94M56459, located in Bldg. 31, Rm. 17. 3. PARTS: Parts shall be provided by the Contractor at no additional charge. Any parts utilized will be of equal or exceed in quality to those parts used by original equipment manufacturer and will be installed by the Contractor as part of this contract. No modification of the equipment shall be permitted without the prior approval of the Contracting Officer?s Technical Representative (COTR) of the Research Office. The Research office is located in Building 22, Room G-20C of the VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. Phone: 734-769-7100 ext. 5603. 4. SERVICE CALLS: a. Hours of work: All scheduled work will be performed during normal working hours of 8:00 a.m. until 4:30 p.m., Monday through Friday, excluding holidays, or as arranged with the COTR. The ten holidays observed by the Federal Government are New Year?s Day, Martin Luther King Day, President?s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday. b. Emergency Service: Emergency service calls are ones required when the equipment is not in a usable conditions and immediate response by the Contractor is required. Response time of the Contractor for emergency service calls will be four (4) working hours after official notification. The term ?response time? will result in the physical presence of a fully qualified technician arriving on station within the specified time. Equipment to be fully operational within 24 hours of on station response time. If Contractor requires additional parts/time to complete required repair/service, Contractor should contact the COTR to coordinated completion of work within an agreed upon completion time/date, to keep inoperable time at an absolute minimum. All repairs shall be in accordance with accredited standards and codes applicable to equipment covered in this contract. Equipment shall meet or exceed manufacturer?s operating specifications upon completion of repairs. 5. PREVENTIVE MAINTENANCE: Contract shall perform four (4) scheduled preventive maintenance (PM) procedures during the contract year during the month(s) of November 2005, February, May, and August 2006. It is the sole responsibility of the Contractor to schedule and perform all preventive maintenance inspections. The specific dates for scheduling the (PM) will be coordinated between the Contractor and the COTR of the Research Office. Preventive maintenance inspections will be performed during the hours of 8:00 a.m. until 4:30 p.m., Monday through Friday, excluding holidays and any day declared an official Federal Holiday. Holidays observed by the VA Ann Arbor Healthcare System are as previously referenced in #4 Service Calls, section (a). Preventive maintenance procedures shall include all items outlined in the original equipment manufacturer?s (PM) procedures. The Contractor shall submit a copy of the proposed procedures thirty days after award of the contract. Preventive Maintenance will include, but not be restricted to: a) Cleaning of the equipment; b) Completing installation and documentation of all minor operational and reliability engineering notice changes; c) Running system diagnostics, hardware and software; d) Calibrating and lubricating the equipment to manufacturer?s specifications; e) Providing all factory required or recommended software updates. The Contractor is responsible for incorporating all manufacturer recommended hardware and software updates which ensure performance to current product specifications. This service will be performed during routine preventive maintenance, or at a time frame as specified by the manufacturer or mutually agreed upon by the VAAAHS and the Contractor. All will be provided by the Contractor at no additional cost to the VAAAHS. Documented form showing the (PM) procedures followed is required by the Contractor. 6. SPECIAL CONDITIONS: a. The Contractor?s service representative shall notify the designated COTR of the Research Office, located in Bldg. 22, Room G-20C, VAAAHS, 2215 Fuller Road, Ann Arbor, MI 48105, 734-769-7100 ext 5603 upon arrival at the facility and prior to beginning work and upon completion of work. b. Clean-up: Contractor shall leave the work site in as clean a condition as when work began. Debris accumulated during work procedures shall be removed and the equipment or furniture moved during work procedures shall be repositioned to original locations. c. Removal of Equipment by Contractor: Any equipment required to be removed from this facility for repair work shall not be removed without the proper approval from the Acquisition and Materiel Management Service representative. 7. SERVICE TICKET: Contractor shall submit to the designated COTR of the Research Office a routine work and/or emergency work slip. This work slip is to be legibly signed and dated by the VA technician who operated the equipment/or works in the department where the work has been completed. Upon completion Contractor will submit this signed service ticket to the COTR of the Research Office. Service ticket shall be legible and contain the following information: a) date and time of service/inspection; b) brief description of work performed; c) equipment type, model and serial number; d) copy of (PM) inspection report, if applicable; e) signature of designated COTR accepting work during normal working hours; and f) on Saturdays, Sundays & acknowledged Holidays observed by the VAAAHS and after normal working hours, the Contractor will notify the originator of the call, upon completion of service call. Upon completion service ticket should be dated and signed by personnel on duty at time of work/repair. Service ticket must be forward to the COTR of the Research office. After hours service will be provided at an additional charge. 8. SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer or his designee may determine it be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer or his designee will notify the Contractor of any non-compliance with the foregoing provisions and the actions to correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his representative at this site of work, shall be deemed sufficient for the aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work and hold the Contractor in default. The following clauses apply to this solicitation: 52.212-4 Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Order Commercial Items with (b) (1), (7), (16), (17), (18), (19), (20), (26), and (32). The following provisions apply to this solicitation: FAR 52.212-1 Instructions to Offeror-Commercial Items, 52.212-3 Offereror Representations and Certifications-Commercial Items, FAR 52.217-8 Option to Extend Services (This option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months); FAR 52-217-9 Option to Extend Term of the Contract (This option may be exercised by written notice to the Contractor within 30 days provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires.); FAR 52.222-48 Exemption from Application of Service Contact Act Provisions for Contacts for Maintenance, etc.; FAR 232-18 Availability of Funds, FAR 52.233-2 Service of Protest, VAAR 852.233-70 Protest Content, VAAR 852.233-71 Alternate Protest Procedure; VAAR 852.270-1 Representative of Contracting Officers and VAAR 852.270-4 Commercial Advertising. Quotes will be evaluated on the following criteria according to FAR 52.212-2(a): Technical/service capability of equipment and past or referenced performance, when combined are equal to price. Award will be made to the offeror who meets the requirements above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the price information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a complete copy of the provisions 52.212-3 Offeror Representations and Certifications?Commercial Items (with signature), the completed certification at 52.222-48, literature/brochures on proposed service maintenance and the company DUNS number. Quotations and all required information must be received by 3:30 p.m. EST September 29, 2005. Quotations shall be submitted to Cynthia W.
