Description
The Hefner VAMC, Salisbury, NC is seeking profesional architect-engineer services to design the addition of 4,500 S.F. of new floor space and 8,705 S.F. of renovated floor space on the first floor of Building 2 and 4,500 S.F. of new floor space and 6,428 S.F. of renovated floor space on the second floor of Building 2. The space is intended to include the Dental Clinic, Pharmacy Service, Imaging Service and the Endoscopy and Urology Clinics. The construction project is a Phase I - 9,000 square feet two story clinical addition to the main hospital here in Salisbury, North Carolina. The addition would be for expansion of Specialty Care and Ancillary/Diagnostic services. The new 2-story addition will be constructed on top of the single story addition on the back side of Building 2 which currently houses the SPD operations. The project will need to be designed to accommodate the Phase II addition which will construct an addition two floors on top of the phase I addition. Roofing, Mechanical and other systems will need to be designed to allow for the Phase II addition minimizing waste to the greatest extent possible. Planning will need to accomplished to facilitate the optimal layout, traffic flow patterns and openings between the new and existing space. There has been explosive growth in the need for Primary Care services at Salisbury in the past several years. National statistics as of July 2004 show that Salisbury VA is #1 in the Nation for percentage increase in new uniques and #2 in the Nation in the actual number of new uniques. Salisbury VA also leads all of VISN 6 in uniques, accounting for more than 19% of uniques. This is not a request for proposal. This procurement is unrestricted; both large and small businesses may submit two copies each of SF 330 to the below named Contracting Officer no later than close of business (3:30 PM EST) October 20, 2005. The procurement is restricted to firms located within a 150 mile radius of Salisbury (Rowan County) North Carolina. The selected firm shall provide all necessary labor, material, equipment, and architectural and engineering services as required to prepare complete contract drawings, specifications, technical reports, and cost estimates through construction of the project. Design shall be in accordance with all applicable requirements of VA construction standards, space criteria, NFPA, NEC, OSHA and local, state and federal regulations, Reviews at 25%, 75 and 99% are required. All construction documents are to be provided electronically in the current version of AutoCad and MS Word. NAICS Code 541310 is applicable to this procurement. Faxed copies of SF 330 are not acceptable. The form may be downloaded at www.gsa.gov/forms. Government-wide Numbered Note 24 is applicable.
