Description
The Southern Arizona VA Health Care System (SAVAHCS), Tucson, AZ intends to negotiate sole source with Resource Systems, 22 West Main St., New Concord, OH 43762 to procure the Care Tracker System. The system shall consist of the following items: 1. #5707 Enterprise Care Tracker Module: 1 each 2. #5708 Enterprise Kiosk Seat Fee: 12 each 3. # 5749A Posi Flex Touch Screen Kiosk-HD: 12 each 4. #5697 Software support fee: 1 each 5. #5730A Post Flex 3 year Hot-Swap Agreement: 12 each 6. #3983B Implementation Kick-Start Service: 1 each 7. #3965DDD On-site Training (daily rate): 2 days 8. #3985B Estimated travel expenses: 1 each The Care Tracker System is an off-the-shelf commercial item providing patient point of care tracking which allows clinical managers a comprehensive and accurate view of quality indicators and quality measures with an easily accessible and understandable system. Additionally, the system provides report alerts to nursing staff advising of potential patient care problems and complete security and privacy of patient records. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #2005-05. The full text of all provisions and clauses cited herein may be obtained from the following URLs: http://www.arnet.gov/far http://vaww1.va.gov/oamm/vaar/ The following provisions apply to the solicitation: FAR 52.212-1 Instructions to Offerors-Commercial Items; FAR 52.212-3 Offeror Representations and Certifications-Commercial Items; The following clauses apply to the acquisition: FAR 52.212-4 Contract Terms and Conditions-Commercial Items; Addendum to 52.212-4: 52-204-7 Central Contractor Registration; VAAR 852.270-4 Commercial advertising. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. The following FAR clauses within 52.212-5 apply: 52.222-19 Child Labor?Cooperation with Authorities and Remedies; 52.232-34 Payment by Electronic Funds Transfer?Other than Central Contractor Registration. This notice of intent is not a request for competitive proposals. Interested parties may identify their interest and capability to respond to the requirement or submit proposals, which may be faxed to (520) 629-1817 or e-mailed, no later than close of business (4:30PM local time) on September 26, 2005. A determination by the Government not to compete this proposed action based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
