Description
COMBINED SYNOPSIS: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being accepted and a written solicitation will not be issued. Solicitation Number F3P3E95068A100 is issued as a request for quotation. This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 01-14. The SIC Code is 7699, size standard is $6.0 Million and the NAICS code is 811490. The contract is for the purchase of preventive and emergency maintenance/ repair of Fitness Equipment at the gym located in Randolph AFB, TX in accordance with the attached Statement Of Work. The period of performance shall be six months from date of award. Award of Firm Fixed Price Contract is anticipated by 1 Oct 05. Full text of the following clauses and/or provisions can be viewed through Internet access at the AF FAR Site, http://farsite.hill.af.mil. FAR 52.212-1, Instructions to Offerors-Commercial; FAR 52.212-2, Evaluation ? Commercial Items, are applicable to this acquisition. Specific evaluation criteria for this provision are as follows: (1) conformance to the Statement of Work (Technical Proposal), (2) past performance and (3) price. Factor one (1) has a greater weight than factor two (2). Factor two (2) has a greater weight than factor three (3). Offers submitted shall address the following: conformance to the SOW/technical proposal specifications, past performance, and price. Offerors must also include a completed copy of the Offeror Representations and Certifications ?Commercial Items (FAR 52.212-3), which can be obtained at http://www.arnet.gov/far. All responses that meet the criteria contained herein will be considered. ?Or equals? will not be accepted. Award will be made using Simplified Acquisition Procedures, therefore, the evaluation procedures at FAR 13.106-2 will be used. All responsible sources may submit an offer, which, if timely, will be considered by this agency. Description of Work: Preventive and Equipment Maintenance 1.1. The contractor will perform the following services on the Fitness Center Equipment. The contractor shall provide all personnel, equipment, tools (this includes special manufacturer?s tools), materials, supervision, and other items necessary to inspect and repair equipment as defined in this Statement of Work (SOW) at Randolph Air Force Base, Texas. 1.2. The contractor will submit, and state clearly, all repairs and parts estimates in writing to the Fitness Center Director or appointed representative (APR) prior to performing repairs and ordering equipment. The Fitness Center Director or appointed representative will approve all repairs and parts orders, in writing, prior to beginning work. All replacement parts required for maintaining equipment will be ordered within 48 hours after service call, provided the contractor received approval by the FCD or appointed representative. 2. Personnel 2.1. Contract Manager: The contractor will provide a contact manager who shall be responsible for the performance of the work. The names of an alternate(s) shall be provided to the Fitness Center Director or appointed representative in writing and shall act for the manager in the absence of the contractor. 2.2.The government is authorized to restrict the employment under this contract of any contractor employee, or respective contractor employee who is identified as a potential threat to the health, safety, and security, general wellbeing, or operational mission of these facilities or its patrons. 2.3. The contractor will provide current certifications stating that Service Technicians are qualified to service and maintain the newest Fitness equipment on the floor at the beginning of the contract year. The contractor is responsible to provide to the FCD or APR documentation of participation in Continuing Education workshop for Fitness Equipment repair for his/her Service Technician. At least one workshop must be completed within a contract year. 3. Quality control The contractor will ensure that all work performed under this SOW meets the Quality Standards outlined in the 2005/2006 SOW. 4. Hours of Operation 4.1. The contractor shall conduct maintenance and repairs between 5:00 AM and 11:00 PM Monday through Friday, excluding all federal holidays. The Fitness and Sports Director or an appointed representative must approve work performed before the Hours of 8:30 AM and after 5 PM in advance. The contractor will upon arrival sign in and sign out before leaving the facility. A fitness center staff member will initial to verify the entry and exit. In addition the Contractor will provide a written overview on all the work accomplished, and request for parts within 24 hours. 4.2. The contractor will coordinate all preventative monthly and quarterly Maintenance inspections with the Fitness Center Director or appointed representative at least 15 calendar days in advance. Monthly service will be conducted within the first five days of each month. 5. Government Furnished Supplies and Service The Fitness Center Director will appoint Fitness Center staff members authorized to sign all service reports (statement or ticket clearly explaining the work performed), thus verifying the work is completed and acknowledging satisfaction of services performed. 6. Contractor Furnished Parts and Materials 6.1. The contractor will provide FCD or APR a price list of prices required, for equipment repair (within 48 hours), prior to servicing. Once replacement parts are approved by the Fitness Center Director or appointed representative, the contractor shall provide all parts on a reimbursable basis, unless the parts are available in the activity?s supply. All replacement parts will be new factory warranty. Any rebuilt parts must be approved by the FCD or APR and must be purchased from the manufacturer. Parts under warranty will order and returned to the manufacturer by the Fitness Center staff. The Contractor will ensure parts under warranty are signed over to FCD or APR within 24 hours. The Government, Fitness Center Director or appointed representative reserves the right at any time to provide contractor with government purchased replacement parts. 6.2. The contractor will provide documentation of the origin of parts provided and an invoice to the Fitness Center Director or appointed representative of all maintenance parts purchased within 24 hours to be reimbursed by the government. 7. Specific Tasks 7. The contractor will provide scheduled preventive maintenance inspections (PMI?s) IAW manufacturer?s specifications. The contractor will respond to un- scheduled Emergency service calls within 48 hours. Emergency calls are paid per call. 7.1 PMI?s are the care and servicing of equipment for the purpose of retaining it in a serviceable condition IAW manufacturer?s maintenance specification literature. Inspection consists of ensuring proper, noiseless, clean, and safe operation, detection and correction of incipient failures thus helping prevent or reduce equipment downtime. The PMI?s should be accomplished within the prescribed time frame recommended by the manufacturer. The written status report for PMI?s will clearly describe all work completed on each piece of equipment and state procedures and future work if necessary. The report will be submitted to the FCD or APR 48 hours after the PMI is completed. In addition, the contractor will perform quarterly, bi-annually or annually preventive maintenance IAW manufacturer recommended procedures for the specific piece of equipment. Monthly PMI?s will not be neglected because of above mentioned maintenance requirements. This includes, but is not limited to actions pertaining to periodic technical inspections lubrications, adjustments, calibration, interior cleaning, and replacement of worn or deteriorated parts and repair/replacement of torn or worn upholstered pads, handgrips and slip protection. 7.2.1. Corrosion control-minor repair will be accomplished inside the fitness centers. Task will consist of removing rust and repainting minor affected areas on equipment (i.e. base of bikes, treadmills, cross trainers, stair steppers, weight benches, etc). Paint must be approved by Fitness Center Director or appointed representative. 7.2.2. The contractor will dispose of all unserviceable parts with the permission of the Fitness Center Director or appointed representative immediately after they have been replaced. Instructions for replacement of warranty parts see paragraph 6.1.. 7.3. An equipment maintenance logbook accessible to the contractor and the Fitness Center Organization must be provided by the contractor IAW Government requirements. The logbook must be automated and accessible at any time. The Web based operation must be established and maintained at all times. All work performed on each piece of equipment must be documented by the contractor. Each piece of equipment is to have it?s own history/data sheet that will be provided and updated by the contractor. Any work performed by the Fitness Center staff will also be documented. In addition the log will show information such as mileage, or other means of showing usage by month on each cardio machine. The log book will also show ordering and status of parts and broken equipment. A hard copy of each equipment dated the first of each month must be provided for inspection purposes to the Fitness Center Director or appointed representative. 7.4. The contractor?s service technician will complete and meet Manufacturer?s specifications on all repairs in a timely manner with the exception of delays due to replacement parts delivery. 7.5. The contractor?s service technician will prepare clear and dated emergency service call documentation (scheduled or unscheduled) providing information on the work performed and recommendations. Contractor will obtain a signature from the appointed, authorized Fitness Center Staff verifying that the service was completed to their satisfaction. 8. Uptime is defined as the time equipment is performing according to manufacturer?s specifications. Downtime is defined as the time where equipment is not operational due to breakdown, ongoing corrective maintenance or failure to perform according to manufacturer?s specifications. Downtime should not exceed 72 Hours. 10. Equipment 10.1. The contractor will provide service following Manufacturers specification for the equipment in p. 10.3. 10.2. To perform monthly maintenance PMI?s, the FCD or APR is required to provide an Equipment service manual to the contractor for individual equipment (same equipment one manual). The contractor and FCD or APR is required to read, date and sign that he/she fully understands the preventive maintenance manuals requirement for each machine on the floor. A copy of each manual and signatures will be filed by the FCD with a copy of the contract. 10.3. The monthly PMI?s and other scheduled preventive maintenance and Emergency calls will include but are not limited to approximately a hundred cardio machines s.a. treadmills, cross trainers, specialty equipment, upright and recumbent bikes and group cycling bikes. At least four circuits of selecterized weight machines, plate loaded and free weight equipment. In addition to approx. hundred attached TV screens and programmable audio boxes.
