Description
This is a COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE INFORMATION IN far Subpart 12.6, using Simplified Acquisition, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued PAPER COPIES OF THIS SOLIC1TATION WILL NOT BE AVAILABLE. The RFQ number is N00259-05-T- 0515. This solicitation documents and incorporated provisions and clauses in effect through FAC 2005-05. It is the contractor?s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses; www.arnet.gove/far and www.dtic.mil/dfars. The NAICS code is 621511. This is an unrestricted action. Naval Medical Center San Diego, Material Management Division request responses from qualified sources capable of providing NEWBORN SCREENING SERVICES in accordance with the Statement of Work (SOW). 1. Base Year, Period of Performance (POP) 10/01/2005 ? 09/30/2006; Qty: 1 Year, U/P: _________, Extended Amt: ___________ 2. Option Year One, POP: 10/01/2006 ? 09/30/2007; Qty: 1 Year, U/P: ________, Extended Amt:_________ 3. Option Year Two, POP: 10/01/2007 ? 09/30/2008; Qty: 1Year, U/P: ________, Extended Amt:_________ STATEMENT OF WORK GENERAL REQUIREMENTS: The effort required hereunder shall be performed in accordance with this Statement of Work and in accordance with all other terms and conditions set forth herein. The contractor shall provide all services, materials and equipment necessary for the service of Pediatric Newborn Screening tests, Located at Naval Hospital Lemoore, California to ensure dependable and reliable operation of the equipment. The scope of work performed under these specifications includes the furnishing of all supplies, shipping and testing requirements to assure continued operation at their designed efficiency and capacity. THE CONTRACTOR SHALL PERFORM THE FOLLOWING FUNCTIONS: STATEMENT OF WORK FOR PEDIATRIX SCREENING, INC. Definition of Service Comprehensive Newborn Screening program (see disorder list) is optimally collected in the immediate newborn period >24-48 hours of life, but can be ordered for a baby or child of any age. 2nd Tier DNA Molecular Analysis: Pediatrix Screening has also integrated the use of DNA based testing into newborn screening. This mode of testing provides accurate, timely and clinically important information for the health care provider caring for a newborn affected with a disorder. A.DESCRIPTION OF SERVICES. 1.Contractor shall test all samples for the following: a.Biotinidase Deficiency b.Congenital Primary Hypothyroidism c.Galactosemia d.Sickle Cell Hemoglobinopathies e.Medium Chain Acyl Co-A Dehdrogenase Deficiency f.Phenylketonuria g.Cystic Fibrosis h.Congenital Adrenal Hyperplasia i.Glucose-6-Phosphate Dehydrogenase Deficiency 2.Additionally, contractor shall test, upon request, all samples for the following: a.Organic Acid Dissorders b.Fatty Acid Oxidations Disorders c.Amino Acid Disorders d.Other Abnormal Profiles 3.All Reflex DNA testing, RE-screening tests are done at no charge to NH Lemoore. 4.Results Reporting On-line Results are made available via secured internet access for all screening results All abnormal results are reported by Board Certified Genetic Counselors Genetic Counselors also provide education and referral support to health care providers for patients with suspected disease Abnormal results are communicated by telephone and fax to health care provider 24 hours a day, 7 days a week Provide report feedback for quality assurance of newborn screening process Provide state reporting where applicable 5. Contractor shall notify Naval Hospital, Lemoore of all significant abnormal results. 6.The Contractor shall provide and pay for the transportation of specimens for testing from the Naval Hospital, Lemoore daily, Monday through Friday, (Saturday where available) at approximately the same time each day via overnight shipping. 7.The Period of performance shall be from 1 Jun-05 - 1 Jun-08. B.TYPES OF SUPPLIES - GOVERNMENT PROVIDED SERVICES AND SUPPLIES: 1.The Government shall complete and submit the Contractor?s test requisition form to the Contractor with each applicable specimen. C.TYPES OF SUPPLIES - CONTRACTOR PROVIDED SERVICES AND SUPPLIES: 1.The Contractor shall continuously provide a sufficient quantity of test requisition forms and filter papers. The Government estimates that 50 test requisition forms and filter papers will be required monthly. 2.The Contractor shall submit test results to Naval Hospital, Lemoore via facsimile and web-based interface, or as designated on the requisition form. 3.The Contractor shall provide critical test results via telephone to the Government point of contact and the ordering physician of record at the telephone number specified on the requisition form. D.SALES SLIPS/DELIVERY TICKETS/INVOICES: 1.The Contractor shall submit a bi-monthly invoice of services to include the following information: a.Subject statement shall include, for each requisition form received and processed, the i.Patient name, ii.Date of drawing of specimen by Government iii.Date of receipt of specimen by Contractor. 2.Note: The contractor shall not invoice for services that are not identified in this agreement. If it is so found that additional items are required under this agreement, a modification will be issued with a new approved price list with the additional items identified. 3.An itemized invoice shall be submitted for each call or at least monthly (all calls for the month). E.SALES TAX EXEMTION: The U.S. Government is exempt form paying sales tax under the__________________. F.PRICE LIST: The contractor is required to submit one original price list. Price list will specify the period of time it is effective. Revised price list will not become effective until approved by the Contracting Officer and a modification will be issued to reflect the changes; therefore the contractor is required to submit a revised price list a minimum of ten (10) calendar days prior to the expiration date. The contractor shall submit a price list detailing out the pricing for requested services. G.TURN-AROUND-TIME Turn-around-time is also an important metric in newborn screening. This represents the time from receipt of the infant?s sample in the lab to the release of results. Our most recent measurements indicate an average turn-around-time of < 1.8 days, and we continuously strive to improve that number. For all sample results are available in less than 3 business days. Keeping the turn-around-time short ensures that important tests results are available as soon as possible to health care professionals and families so that timely therapy can be started. H.
