- The Southern Arizona VA Health Care System (SAVAHCS), Tucson, AZ intends to negotiate sole source with Reliable Medical Products, 301 W. Deer Valley Rd., Suite 2, Phoenix, AZ 85027, as an Arizona authorized dealer, for the following equipment: 1. Versatile 1 Wound Vacuum System: 10 each 2. Universal Holder/Mounting Bracket: 10 each 3. Versatile 1 Stand/Trolley: 10 each The Versatile 1 Wound Vacuum System is the only known pump that offers a disposable suction mechanism for economical replacement and to meet infection control needs. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #2005-05. The full text of all provisions and clauses cited herein may be obtained from the following URLs: http://www.arnet.gov/far http://vaww1.va.gov/oamm/vaar/ The following provisions apply to the solicitation: FAR 52.212-1 Instructions to Offerors-Commercial Items; FAR 52.212-3 Offeror Representations and Certifications-Commercial Items; The following clauses apply to the acquisition: FAR 52.212-4 Contract Terms and Conditions-Commercial Items. Addendum to 52.212-4: 52-204-7 Central Contractor Registration; VAAR 852.270-4 Commercial advertising. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. The following FAR clauses within 52.212-5 apply: 52.222-19 Child Labor?Cooperation with Authorities and Remedies; 52.232-34 Payment by Electronic Funds Transfer?Other than Central Contractor Registration. This notice of intent is not a request for competitive proposals. Interested parties may identify their interest and capability to respond to the requirement or submit proposals, which may be faxed to (520) 629-1817 or e-mailed, no later than close of business (4:30PM local time) on September 27, 2005. A determination by the Government not to compete this proposed action based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
