Description
This is a combined synopsis/solicitation to negotiate a Firm Fixed Price type contract for yearbooks. Request for Proposal 595-28-05 is issued in accordance with FAR Subpart 12.6-Streamlined Procedures for Evaluation and Solicitation for Commercial Items. This announcement incorporates provisions and clauses in effect through FAR FAC 2005-05 and VAAR 97-10. The NAICS Classification code is 511199 All Other Publishers with a size standard of 500 employees. This BOS announcement/solicitation is solicited unrestricted and constitutes the only bid document that will be issued. The contractor shall prepare a written history of the Lebanon VA Medical Center from approximately 1947 to the present. The project should cover the construction and opening of the Lebanon VA through the present. The project will be organized chronologically. It will consist of descriptive text, stories, vignettes, pictures, and other material that will provide an attractive and accurate portrayal of the history of the Lebanon VA Medical Center. While the project will not attempt to be comprehensive, it will highlight the important changes in the Lebanon VA, and the significant role played by the Lebanon VA in helping veterans. It will spotlight various items such as the first veterans admitted to the Lebanon VA. The evolution of the Lebanon VA Medical Center should be the overall theme, with particular attention paid to the veterans themselves. The principal result of this project will be a one-volume work, 80-100 double-spaced, typescript, manuscript pages, exclusive of pictures and other non-textual material. The final product will include, as appropriate, introductory material (preface and foreword), table of contents, text, citations, bibliography, and annexes or appendices, maps, pictures, and a graphics plan, all of which will reflect the highest professional historical standards. The final product will be also be a hard back and bound. The contractor will be required to provide 2000 copies to the Lebanon VA Medical Center. All text material will also be available on disk in an electronic format compatible to VA IT standards. The contractor shall be prepared to have draft chapters reviewed by the Lebanon VA Medical Center and others as deemed appropriate by the Lebanon VA Medical Center. Performance will be incomplete unless and until the final product has been submitted by the contractor and accepted by the Government. The following provisions are incorporated by reference: 52.212-1, Instructions to Offerors - Commercial Items, the following instructions are added as an addendum to this clause: Submit offers in two separate volumes as follows: Volume I, Technical Proposal - Include all technical information required for evaluation, excluding any reference to cost/price. Divide the technical volume into the following areas, Qualifications/experience of staffing proposed and Past Performance. Demonstrate compliance with all minimum technical requirements. Past performance _ include a minimum of 3 points of reference for customers of comparable size, and whose requirements are similar in nature. Volume II, Price Proposal - Pricing should be provided on a per book basis. In accordance with FAR 52.212-2, Evaluation_Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Qualifications/experience of the staffing proposed (2) Past Performance and (3) Price. These factors are stated in their descending orders of importance. The requirement of FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (MAR 2005) must be submitted with the offer. FAR 52.212-4, Contract Terms and Conditions - Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? are hereby incorporated. The following FAR Clauses under FAR 52.212-5 are applicable to this acquisition: 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, and 52.232-34.
