- This is a combined synopsis/solicitation for the purchase of Grease Trap Maintenance. Purchase Request Number F3P3A65075A100. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-20. Award will be made using Simplified Acquisition Procedures, therefore the evaluation procedures at FAR 13.106-2 will be used. The vendor shall follow the attached Statement of Work when completing bid. The NAICS Code is 562998; size standard $6,000,000 dollars. A firm-fixed price award will be made to the responsible offer or submitting the quote that provides the best value to the Government. All responses will be considered, but must be submitted on the requirement included in this synopsis. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the DoD Central Contractor Registration (CCR) database, NO EXCEPTIONS. To register, you may apply at http://www.ccr.gov/ . Responses to this notice shall be provided in writing via e-mail to 2LT Terence Balmaceda at terence.balmaceda@randolph.af.mil no later than 27 SEP, 2:30 P.M. (CT). Offerors must also include a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications?Commercial Items which can be obtained at http://www.bpn.gov/ . Service Contract Act applies please reference wage determination No: 94-2521 REV (29) Area: TX, San Antonio at www.wdol.gov STATEMENT OF WORK 1. DESCRIPTION OF WORK: a. Scope: With the exception of those items listed in paragraph 11 as government furnished utilities and services, the contractor shall provide all personnel, equipment, tools, chemicals, supervision and other items necessary to perform the cleaning and servicing of grease interceptors as defined in this statement of work (SOW) at Randolph Air Force Base TX (RAFB). b. Location: Buildings 468, 500, 860, 1039, 1075, and 1300. 2. PERSONNEL: a. Contract Manager: The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate(s) who shall act for the contractor when the manager is absent shall be provided to the 12 MSG/CECJ Quality Assurance Personnel (QAP). b. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to the operation of this contract. c. The contract manager and alternate(s) must be able to read, write, speak, and understand English. d. The contractor shall provide the QAP the name and phone number of the person who will be called for emergency services. e. Employees: The government is authorized to restrict the employment under this contract of any contractor employee, or respective contractor employee who is identified as a potential threat to the health, safety, security, general well being, or operational mission of this installation and its population. f. Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name. g. The contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval. h. The contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment. Military reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor?s ability to perform. Their absence at any time shall not constitute an excuse for non performance under this contract. 3. QUALITY CONTROL: The contractor shall insure that all work performed under this SOW meets the quality established in paragraph 11. QUALITY ASSURANCE: The government will evaluate the contractor?s performance under this contract using inspections. All surveillance observations will be recorded by the government. When an observation indicates unacceptable performance the QAP will request the contractor?s representative re perform services before the service ticket is signed by the QAP. 4. PHYSICAL SECURITY: a. The contractor shall obtain vehicle passes for each vehicle used on RAFB during the performance period. The necessary applications for vehicle passes can be obtained by contacting the Security Forces Office, (210) 652 3939. The completed applications are then processed through Security Police, Building 1021, RAFB. All vehicle passes must be obtained prior to the start of this contract. b. The contractor shall obtain personnel identification cards for each employee working on this contract. The necessary applications for personnel identification cards can be obtained through the Security Forces Office, (210) 652-3939. The completed applications are then processed at Consolidated Base Personnel Office identification section, Building 399. All personnel identification cards will be obtained prior to the start of this contract. c. The contractor is responsible for recovering and returning to the government any identification card issued an employee upon termination of employment. The contractor will also return to the government all identification cards issued to employees upon completion of this contract. d. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured. e. The contractor shall establish and implement methods of ensuring that all keys issued to him or his employees by the government are not lost or misplaced. The contractor shall report the occurrences of any lost key(s) to the QAP. In the event keys, other than master keys are lost, the contractor will be required, upon direction by the QAP, to re-key or replace the affected lock or locks. However, the government, at its option, may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the government, the total cost of re-keying or the replacement of the lock or locks shall be deducted from the payment due the contractor. In the event a master key is lost, all locks and keys for that system shall be replaced by the government and the total cost deducted from the payment due. No keys issued the contractor by the government shall be duplicated. f. The contractor shall prohibit the use of any government issued keys by any persons other than the contractor?s employees. The contractor shall prohibit the opening of locked areas by his employees to permit entrance of persons other than the contractor?s employees engaged in the performance of assigned work in those areas. g. When the contractor is given the numbers to any combination lock; he shall ensure that those combinations are not revealed to unauthorized persons. 5. HOURS OF OPERATION: a. Normal Hours. The contractor shall begin pumping, cleaning and servicing of grease traps at 7:30 am. b. Recognized Holidays. The contractor is not required to provide service on the following holidays: New Years Day, January 1st Martin Luther King Birthday, 3rd Monday in January Presidents Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4th Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veterans Day, November 11th Thanksgiving Day, 4th Thursday in November Christmas Day, December 25th NOTE: Any of the above holidays falling on a Saturday will be observed on the preceding Friday. Holidays falling on a Sunday will be observed on the following Monday. c. Emergency Service: On occasions, services may be required to support emergency situations outside normal duty hours described above. The contractor?s responsibilities for emergency support are described in paragraph 11. 6. INSPECTION AND ACCEPTANCE OF SERVICES: a. The office having responsibility for inspection and acceptance of work is the 12 MSG/CECJ. b. The office designated as having the responsibility for certification of payment is 12 MSG/CECJ. The Contractor will forward an information copy of all tickets/invoices for the month to the 12 MSG/CECJ QAP no later than the 5th calendar day of the following month in order to insure the contractor is paid in a timely manner. The tickets/invoices will be mailed to the following address: 12 MSG/CECJ 1651 5th Street West Randolph AFB, TX 78150 4513 7. SAFETY: a. The contractor shall work in strict accordance with the standards set forth by NFPA 96, Chapter 8 3, 1991 or the current version, state and local applicable accredited standards and codes. b. The contractor shall be prohibited from transporting to or using on the premises any detergent, solvent, decarbonizer or other cleaning agent categorized as flammable, combustible or explosive material. 8. CONSERVATION OF UTILITIES: The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions, which preclude the waste of utilities. 9. GOVERNMENT FURNISHED UTILITIES AND SERVICES: a. The government shall provide without cost to the contractor, the utilities necessary to accomplish the tasks specified in paragraph 10. b. The government shall provide without cost to the contractor, security police and fire protection. In the event that security police or fire protection is needed by the contractor while on Randolph Air Force Base, the contractor shall call the following numbers: Security Police Emergency 911. Fire Department Emergency 911. c. 12 MSG/CECJ will provide the contractor with a list of building managers and phone numbers on the contract start date. 10. CONTRACTOR FURNISHED EQUIPMENT, PARTS, MATERIALS AND SERVICES: a. Except for the utilities or services specifically stated to be government furnished in paragraph 9, the contractor shall furnish everything required to perform in accordance with this SOW. b. All contractor furnished chemicals, materials etc. must be environmentally safe and approved through the environmental office, 12MSG/CEV prior to the start of the contract. 11. SPECIFIC TASKS: a. The contractor shall clean and service grease interceptors in March and September at the locations described in paragraph 1b, in this SOW. Any additional cleaning of the traps will fall under emergency service, see paragraph 5c. b. The cleaning shall consist of removing the covers and thoroughly cleaning the inside of the grease interceptors to include baffles, inlet connection, outlet connection, walls, and any other unit components. c. The cleaning shall be accomplished by scraping, use of wire brush, chemicals and pumping out. d. At the completion of cleaning all surfaces to include walls, baffles, inlet connection and outlet connection shall be free of any grease buildup. e. Extreme care shall be taken to insure that no grease or sludge is washed out of the outlet connection. f. Adequate ventilation shall be maintained at all times and precautions against the presence of explosive vapors shall be taken. g. The contractor shall be responsible for insuring that all locks and/or safety bars are not damaged by his personnel. The contractor shall be responsible for the replacement of any locks, and for repairing any safety bars damaged by his personnel. h. The work shall be accomplished in such a manner as to minimize the possibility of damage to government property, loss of production time and to safeguard the health and welfare of personnel. i. Ground surface and pavement which has been disturbed by the work shall be restored to its original condition. j. The work area shall be left clean and all debris resulting from this work shall be removed in accordance with state and federal regulations, from RAFB by the contractor. k. Waste Measurement: The contractor shall provide a measuring device, either manually or a meter to accurately determine the amount of waste removed either from each tank or all tanks combined. 1. The contractor shall dispose of liquid waste generated on RAFB at an approved disposal site IAW federal, state and local laws. m. The contractor shall provide the QAP with a completed copy of the manifest for hauled liquid waste to include the gallons and signature of disposal plant operator for each load that is removed from RAFB. These copies will be attached to the invoice/tickets indicated in paragraph 6b of this SOW. n. The contractor shall provide emergency cleaning service within 2 hours after being notified by the QAP. The name and phone number of the person responsible for providing emergency service will be provided to the QAP on the contract start date. o. The contractor must notify the 12 MSG/CECJ, QAP, 72 hours prior to servicing grease traps. p. The contractor shall prepare a service report/ticket each time service is rendered. Contractor shall obtain the signature of the inspector on the report and provide the inspector a copy of the service report/ticket. q. The CO is the only person authorized to contractually obligate the government. r. Only the systems listed on this order will be maintained by the contractor. s. In the event the contractor encounters problems that may prevent him from completing his scheduled work, he shall immediately QAP at (210) 652 5655.
