- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is being issued as a Request for Quote under solicitation number M34000-05-T-BMC0. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This acquisition is a 100% Small Business Set-Aside. The North American Industry Classification Code (NAICS) is 423450 and the small business size standard is 500 employees. The government intends to award a firm fixed price contract, as a result of the quotes received. The Regional Contracting Office, Southwest, Marine Corps Base, Camp Pendleton has a requirement for the following item. CLIN 0001 (40 Each) Zoll AED Plus ! Defibrillators. Contractor to provide the following : 1. Zoll AED Plus Defibrillators, Zoll AED Plus- LCD screen showing voice prompt messages, device advisory messages, elapsed time, shock count and chest compression graph. Supplied with public safety pass cover, softcase, operator’s guide. 2. (1) Set of CPR-D Padz- One piece defibrillation and CPR system with compression, depth and rate sensors. Supplied with grove, barrier mask, scissors, razor, wet and dry wipe. Long life armor pack for storage in softpack carry case. 3. (1) Set of Type 123 lithium batterries, qty of (10) with storage sleeve. Zoll data review software for windows network-freedownload. The following FAR/DFAR provision(s) apply to this acquisition: FAR 52.212-1 Instructions To Offerors —Commercial Items (MAR 2000). FAR 52.212-2 Evaluation-Commercial Items (JAN 1999) FAR 52.212-3 Offerer Representations and Certifications- Commercial Items (MAR 2005) FAR 52.212-4 Contract Terms & conditions -Commercial Items (MAY 1999). FAR 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders — Commercial Items with the following applicable clauses for Para (b) apply to this acquisition: (14) (15) (16) (17). The following DFAR Clauses apply to this acquisition: DFAR 252.212-7001 Contract Terms and Conditions required to implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items with the following applicable clauses for Para (b): DFAR 252.225-7001 Buy American Act and Balance Paymens Program (MARDH 1998 and DFAR 252.232-7003 Electronic Submission of Payment Requests (MARCH 2003). All provisions may be accessed electronically at this address: http://farsite.hill.af.mil Proposals are solicited FOB Destination to San Diego, CA. Delivery is required for all items on or before 28 October 2005. The government will award a contract resulting from this solicitation to the lowest priced technically acceptable offer.
