Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. THIS IS AN UNFUNDED REQUIREMENT SUBJECT TO THE AVAILABILITY OF FUNDS. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Purchase Request No. F1A3B45182A600 is a Request for Quotation (RFQ). The contractor shall provide all personnel, transportation, equipment, tools, materials and supervision necessary to provide Inspection, Maintenance and Repair Services for Computerized controls at the Central Heat Plant (Building 18), Bolling AFB, Washington DC. in accordance with SOW and applicable regulations and manufacturers specifications. The statement of work is used as a minimum guide and is not all-inclusive. The basic performance period for this requirement is 01 October 2005 through 30 September 2006, with 4 one year options. Quotations are being requested not later than 26 September 2005, at 3:30 PM. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. The solicitation is issued as 100% Set Aside Services Disabled Veteran Owned Small Business (SDVOSB). If no packages are received from eligible SDVOSB concerns, the solicitation will be set aside 100% for SMALL BUSINESS CONCERNS pursuant to FAR PART 19. A second notice will not be issued. All quotes shall be submitted by close of business on the cutoff date mentioned herein this solicitation to be considered; in the event this requirement is not set-aside for SDVOSB concerns. The associated NAICS code is 238220 with a size standard of $12.0 Million. The resulting purchase order shall be firm-fixed-price. Alternate Point of Contact: Maria Brodie-Williams at (202) 767-8102, e-mail address; maria.williams@bolling.af.mil. 94-2103 DC, DISTRICT-WIDE STATEMENT OF WORK FOR INSPECTION, MAINTENANCE AND REPAIR SERVICE FOR COMPUTERIZED CONTROLS 1. DESCRIPTION OF SERVICES. The contractor shall furnish all management, tools, supplies, equipment, transportation, labor and all items and services necessary to inspect, repair, and perform maintenance of computerized controls at the Central Heat Plant (Building 18), Bolling Air Force Base, DC, in accordance with industry standards and terms and conditions of the contract. 1.1. The work description shall serve as general in information only and shall not be construed to limit the contractor’s responsibility or obligation to comply with the contract documents for Inspection, Maintenance and Service for computerized controls, Building 18, Central Heating Plant, on Bolling AFB, DC. 1.2. Preventive maintenance and calibration shall be performed on the equipment listed in Appendix A, during each scheduled visit. The contractor shall ensure operations of the equipment after maintenance and calibration operations are performed. 1.2.1. The contractor shall provide a checklist for each system inspection and shall submit a copy to the Contracting Officer for Government approval before the first inspection. The checklist shall be used during each system inspection and shall be signed by the contractor after task is completed. A copy of the completed report shall be submitted to the Quality Assurance Personnel (QAP). 1.2.2. A completed report of all tests shall be submitted to the QAP within five (5) working days after each inspection/service visit. Reports will include results and records of condition found, adjustments, calibration sheet repairs or replacement made and any pertinent recommendation for major tear down, repair, overhaul, rebuild, and/or replacement of components. 1.2.3. The contractor shall review operating procedures for computer software and hardware along with plant foreman and system operators. Contractor shall perform required maintenance, diagnostic assistance of system software when unexplained shutdown is experienced and make minor program changes in accordance with Air Force Policy or recommended changes by the plant manager. 1.3. BOILER COMBUSTION ADJUSTMENT, ANALYSIS, AND VISIBLE EMMISSIONS REQUIREMENT. 1.3.1. Contractor shall provide the District of Columbia Air Quality Division with written notification at least ten (10) days before test so that enforcement personnel can observe on site testing in accordance with 20 DCMR 502. Prepare and provide a sampling and analysis plan for the boiler combustion and analysis to submit along with written notification to the District and 11th CES/CEV Air Program Manager. 1.3.2. BOILER COMBUSTION ADJUSTMENT ANALYSIS. Contractor shall adjust combustion on each boiler (CU 18-1, CU 19-3) starting no earlier than January 1 and complete before May 1, to maximize emission of carbon monoxide (CO) and oxide of nitrogen Nox. Adjustments shall be made in accordance with the provision of 20 DCMR 805.5. The annual certification report must provide sufficient data to substantiate that each boiler has been adjusted to applicable regulations. At a minimum, provide for each emission unit: the dates and times the boilers were adjusted; the name, address and telephone number of the tester and adjuster; the load expressed in pounds per hour steam during the test period; and percent oxygen and provide data for CO and Nox (20 DCMR 805.5). 1.3.3. VISIBLE EMISSIONS TEST. Conduct a minimum of one visible emissions test for each boiler between January 1– December 31. Any visible emissions test must consist of 30 minutes of opacity observations, performed by a certified opacity reader using the EPA reference method 9 (see 40 CFR Part 60, Appendix A, method 9). At a minimum the contractor shall provide: the date and time of the test; the name of, address and telephone number of the tester; the certification of the method 9 observer; the emissions units on line at the time of the test; the boiler load expressed in pounds per hour steam during the test period; the fuel fired during the test and 30 minutes of opacity observation; the results of the observation and the applicable requirement. The emissions test shall be performed on each stack while boilers providing flue gas to that stack fire #2 fuel oil (20 DCMR 502). 1.4. INTERRUPTION OF UTILITIES. The contractor shall not interrupt existing utilities in the plant without obtaining written permission from the QAP or Plant supervisor, a minimum of seven days in advance. Request for outage shall be submitted in writing, and shall state the date, time and duration of proposed outages. The contractor shall make all final connections to utility services. 1.5. CLEAN UP. The material and scrap remaining from new work and classified as trash/debris shall be removed from the Base and disposed of by the contractor in a legally acceptable manner. It shall be the responsibility of the contractor to assure proper loading of all vehicles used for transport of materials and debris. The contractor at no additional cost to the government shall accomplish all clean up activities. 2. EMERGENCY RESPONSE. The contractor shall respond to emergencies or calls within two (2) hours. 3. GENERAL INFORMATION. 3.1. PROJECT MANAGER. The contactor shall provide a project manager or person who shall be responsible for the competent performance of the work and has full authority to act on behalf of the contractor. The project manager shall be able to read, write, speak and understand English. The name of this person and an alternate(s), who shall act for the contractor when the manager is absent, shall be designated in writing to the Contracting Officer before the contract start date and updated as necessary. 3.2. EMPLOYEES. Contractor personnel shall present a neat appearance and be easily recognizable while on base. This may be accomplished by wearing distinctive clothing bearing the name of the company, or by wearing appropriate badges, which contain the company name and employee name. The contractor shall be responsible for acquiring an appropriate number of badges to meet his/her needs at the contractor’s expense. The contractor shall also ensure that employees have valid/current driver’s licenses applicable to operate equipment in accordance with state, county, and local laws and regulations. The abrupt absence of contractor employees at any time shall not constitute an excuse for nonperformance under this contract. 3.3. GOVERNMENT FURNISHED ITEMS. Utilities will be provided without charge to the contractor in connection with performance of this contract. These utilities include electricity, water and sewage in the performance of this contract. 3.4. HOURS OF OPERATION. The contractor shall perform the services required under this contract during the following hours: 7:30 AM to 4:30 PM Monday thru Friday, except for Federal Holidays. 3.4.1 RECOGNIZED HOLIDAYS. The following is a list of Federal Holidays observed by the installation: New Year’s Day Labor Day Martin Luther King Jr. Birthday Columbus Day President’s Day Veteran’s Day Memorial Day Thanksgiving Day Independence Day Christmas Day Inauguration Day 3.5. QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance under this contract. For task s listed in the statement of work, the QAP will inspect all services specified. When an observation indicates defective performance, the QAP will notify the contractor manager or representative. The contractor shall ensure re-performance through his or her quality inspection system. 3.5.1. GOVERNMENT REMEDIES. The contracting administrator/officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions-Commercial Items (May 1997), for contractor’s failure to correct non-conforming services. 3.6. SECURITY REQUIREMENTS. 3.6.1. All personnel employed by the contractor in performance of this contract, or any representative of the contractor entering Bolling Air Force, shall abide by all security regulations of the installation. 3.6.2. The contractor shall obtain personnel identification passes for all employees and vehicle passes for all contractor and personnel entering Bolling Air Force Base. The contractor must complete AF Form 75, Request for Visitor/Vehicle Pass, and DD Form 1172, Application for Uniformed Services Identification Card, and submit it through the contracting officer to the Security Forces Pass and Registration. Vehicle registration, proof of insurance and valid drivers license must be presented for all vehicles to be registered. 3.6.3. Lost vehicle and entry passes shall be reported immediately to Security Police, and Quality Assurance Personnel (QAP). 3.6.4. The contractor shall retrieve all identification media, including vehicle passes from employees who depart the company for any reason before the contract expires. 3.7. PERFORMANCE OF SERVICE DURING DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. The contractor shall perform all services under this contract during a national emergency or crisis according to the Department of Defense Instruction (DODI) 3020.37. 3.8. WARRANTY. The contractor shall be responsible for replacing any warranty item within the appropriate period of such warranty. This shall include manufacturer warranties. 3.9. ENVIRONMENTAL CONTROLS. Compliance with Laws and Regulations. The contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, permits, DOD, Air Force, and base environmental requirements and instructions. The contractor shall ensure that all subcontractors comply with the same. The contractor shall ensure policies and procedures are established that protect the safety and health of employees and the community to minimize or eliminate the risk of environmental pollution. If the contractor spills or releases any substance listed in 40 CFR 302 into the environment, the contractor shall immediately report the incident to the Environmental Coordinator at 202-767-8600, in accordance with procedures. The contractor shall notify the primary QAP or in his or her absence the alternate QAP. The contractor shall be liable for containment and environmental clean up of the spill or release of such substance. If spillage creates contaminated soil, the contractor shall at his own expense, remove the contaminated soil, properly dispose of the soil and replace it with uncontaminated top soil and restore the grounds to their original condition. The contractor shall not dispose of contaminated soil or any hazardous materials on Bolling Air Force Base.
