Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Award of the contract may be made without discussions, request that the most favorable pricing be offered. The solicitation number is 541-006-06 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-05. The North American Industry Classification System (NAICS) code is 811310 with a size standard of $6 million. Offeror shall provide all labor, supervision, parts, materials, and equipment necessary to perform preventive maintenance and emergency response, when needed, on the Government-Owned Kitchen Equipment. Offeror pricing shall be as follows: Preventive Maintenance and Repair of Nutrition and Food Service Equipment for Louis Stokes Cleveland, Department of Veterans Affairs Medical Center (LSVAMC), located at 10701 East Boulevard, Cleveland, Ohio 44106 and 10000 Brecksville, Ohio 44141 for the following equipment: CLIN 0001 Therma System Time Captain Controls, include drop cords, 24 each; CLIN 0002 Vulcan, Combi Ovens (include filters); 3 each; CLIN 0003 Cleveland Range, Steamer, 3 each; CLIN 0004 Vulcan Quick Chill, 2 each; CLIN 0005 Therma System, Perfect Temp Carts, 64 each; CLIN 0006 Avtec Conveyor System, 2 each; CLIN 0007 Avtec, Cold Top, 4 each; CLIN 0008 Avtec Hot Well, 4 each; CLIN 0009 Oven, Middleby, 1 each; Hobalt Warming Cabinet, 2 each; CLIN 0010 Enhan Slicer, 12 inch, MDL 1712E-1, 1 each; CLIN 0011 Enhan Slicer, MDL 2912-1, 1 each; CLIN 0012 Enhan Mixer, MDL H600-4, 1 each.. Contact Vito Mazzaro, COTR, at 440-526-3030, Ext. 6156 for a complete list of the equipment. Period of performance shall be a base period plus 2 option years. The initial performance period shall commence from the date of award through September 30, 2006. The performance period shall be as follows: Base Year: Award through September 30, 2006; Option Year 1: October 1, 2006 through September 30, 2007; Option Year 2: October 1, 2007 through September 30, 2008. PREVENTIVE MAINTENANCE INSPECTION: Preventive Maintenance shall be coordinated five (5) working days in advance with the Nutrition and Food Service Supervisor prior to the start of work. All scheduled Preventive Maintenance shall be performed quarterly during the months of January, April, July and October between normal working hours of 8:00A.M. through 4:30P.M. (excluding Federal Holidays). The ten (10) holidays observed by the Federal Government are: New Year's Day, Dr. Martin Luther King Jr.'s Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas, and any other day specifically declared by the President of the United States to be a Federal Holiday. QUALIFICATIONS: All service and preventive maintenance shall be performed by competent, factory trained technicians, experienced and qualified to work on this equipment. Service and preventive maintenance shall be in accordance with commercial practices and equipment per manufacturer's specifications. The offeror's primary place of business shall be within 150 miles of the Wade Park and Brecksville facilities. RESPONSE TIME: Repair service shall be performed within four (4) hours of notification from the Nutrition and Food Service Supervisor or COTR. DOCUMENTATION OF PREVENTIVE MAINTENANCE: Sign in and Sign out: Offeror shall report to COTR, Andrew Conti, 216-791-3800, Ext. 3033, Engineering Service for equipment located at the VA Medical Center in Cleveland, Ohio before starting job and obtain initials of operator when the job is completed. Offeror shall report to COTR, Vito Mazzaro, 440-526-3030, Ext. 6156, Engineering Service for the equipment located at the VA Medical Center in Brecksville, Ohio. Offeror shall provide to the VA written evidence of preventive maintenance completion, to include recommendations of opportunities for improvement or identification of concerns. All documentation, noting services performed and completed shall be provided to VA COTR upon departure from the VA facility. CONTRACTOR PERSONNEL: The Government reserves the right to restrict any contractor employee from performing services under this contract who violates federal regulations or is identified as a potential threat to the security, safety, health or operational mission of the Department of Veterans Affairs Medical Centers and their populations. INVOICES: Invoices shall be submitted after services have been performed and documentation has been received by the COTR. PRICE PROPOSAL: Priced per CLIN per contract year and priced to reflect each option year. The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition with the following changes, items (d), 9e) and (h) of the standard clause is deleted. The provision at FAR 52.212-3, Offeror Representations & Certifications- Commercial Items must be completed and returned by the offeror. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition with the following addenda: FAR 52.215-5, Facsimile Proposals, FAR 52.232-19, Availability of Funds for Next Fiscal Year; FAR 52.217-8, Option to Extend Services (with the following blank filled in option by written notice to the contractor within 30 days); FAR 52.217-9, Option to Extend the Term of the Contract (with the following blank filled in option by written notice to the contractor within 60 days); VAAR 852.270-4 Commercial Advertising, 852.233-70 Protest Content; 852-233-71 Alternate Protest Procedure; 852.237-70 Contractor Responsibilities and, 852-236-86 Worker's Compensation. The clause at FAR 52.212-5, Contract Terms & Conditions Required To Implement Statutes or Executive Orders - Commercial Items applies to this acquisition to include the following clauses: 52.222-21, 52.222-26, 52.222-36, 52.222-37, 52.232-34, 52.222-41 and 52.222-42. Preventive maintenance of the Kitchen Equipment listed above shall be in accordance with industry standards, as well as all applicable Federal, State laws, codes, and regulations in effect at time of award.
