Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region EXAM ROOM INSTRUMENTS: Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Exam Room Instruments, which include Otoscopes, Ophthalmoscopes, Laryngoscopes, Stethoscopes, etc. Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing Exam Room Instruments. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) on Exam Room Instruments, your company is eligible to participate in the standardization process. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than October 4, 2005. For questions, please call (619) 532-8517. 1.Does your company manufacture or distribute exam room instruments (Otoscopes, Ophthalmoscopes, Laryngoscopes, Stethoscopes, etc.)? If so, please list the types/variations for the instrument. 2.Are your products available through the Prime Vendor Program? 3.What is your DAPA number? 4.Under what brand(s) does your company manufacture or distribute your products? 5.Under what standards are your products manufactured? 6.Are your instruments latex free? 7.For each type instrument, please list the models available and their features. 8.For each type of instrument, please list the features that make your instruments precise and durable along with its source of power. 9.What is the warranty on your products? 10.Does your company offer a repair program? If so, what are the terms? 11.Do your products require in-service training? If so, is this training provided by your company at no charge? 12.Does your company provide training for biomedical repair technicians? If so, is this training provided by your company at no charge? Is this training available at the individual medical facilities? 13.Does your company provide customer service 24/7? 14.Is there any history of backorders and/or recalls for this product group?
