Description

DUE: 10/24/2005 POC: Michael I. Heilman, Contracting Specialist Phone: (562)-826-8062 Fax: (562)-961-1378 This is a combined synopsis/solicitation issued for commercial items. The VA Network 22 Logistics Office, Long Beach, California 90822 intends to negotiate a commercial (FAR Part 12) sole source contract with Prosthetic Design Inc. 700 Harco Drive, Clayton, OH 45315 under the authority of FAR 6.302-1. A justification for other than full and open competition has been prepared in support of this acquisition. The Request for Proposal Number RFQ 600-371-06MH entitled Prosthetics & Sensory Aids, with supplies is hereby issued for Commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 2005-06. The NAICS Code is 423450 and the small business size standard is considered 500 employees. The Computer Aided Design/Computer Aided Manufacturing (CAD/CAM) System allows for a 30 minute turn-around in socket fabrication. THIS BOS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION ISSUED. A SEPARATE WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. Included in the price are a one-year warranty, training, technical phone support, on-site technical support, replacement parts and installation. The equipment and supplies are for VA Loma Linda Healthcare System Prosthetic & Sensory Aids Service. The Equipment is: Nexform Carver (Large Servo Carver); Nexcollex (Dust Collector); Nexvac (Automated Thermoformer); Nexbox-Sat (Complete Tool Cabinet); instalation, and training. The initial supplies include Transformer and Profiler Carve Forms; Prosthetic Vise and Phd: Preform Holding Devise. The equipment and supplies shall be delivered to the VA Loma Linda Healthcare System, Warehouse, 11201 Benton Street, Loma Linda, CA 92357. Offeror shall submit a complete copy of provision of FAR 52.212-3, Offeror Representation and Certification-Commercial Items (Mar 2005). The following Federal Acquisition Regulation (FAR) Clauses and Provisions also apply to the acquisition: 52.212-1, Instruction to Offerors-Commercial Items (Jan 2005); 52.212-4, Contract Terms and Conditions-Commercial Items (SEPT 2005); 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items (SEPT 2005) included clauses (b) of 52.212-5 the following apply: 52.203-6, 52.219-4, 52.219-8, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-39, 52.222-48, 52.225-1, 52.225-3, 52.225-13, 52.225-15, and 52.232-34. In addition, incorporated herein by reference are the following: VA Acquisition Regulations (VAAR) Clauses and Provisions: 852.211-70, Requirements for Operating and Maintenance Manuals (NOV 1984); 852.211-71, Guaranty Clause (NOV 1984). Original proposal shall be submitted on company letterhead along with descriptive literature for all items offered to Michael I. Heilman, Contracting Officer (NBC/MP). A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.
