This FEDERAL BUSINESS OPPORTUNITY announcement is a notice that TRICARE Southeast that include all DOD Medical Treatment Faciliti es (MTFs) in the states of Alabama, Florida, Georgia, Mississippi, South Carolina, Tennessee, and Southcom are in the process of standardizing Surgical Masks as listed in the subject line above. Companies must obtain, or be in the process of obtaining, a D istribution and Pricing Agreements (DAPAs) with the Defense Supply Center Philadelphia for these items to be eligible to participate in the standardization process. Please respond by Email: vincent.pacchiana@se.amedd.army.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than the response date of 27 October 2005. For que stions, please call (706) 787-2094. Technical Criteria 1. Does your company manufacture and/or distribute a full line of disposable hypoallergenic surgical masks? If so, please list the types of masks your company offers. 2. Does your company manufacture and/or distribute masks in pediatric/child sizes? 3. Are your products available through the Prime Vendor, Owens & Minor? 4. What is your DAPA Number? 5. Under what brand(s) does your company manufacture or distribute your product? 6. Are your products latex free? 7. Under what standards is your product manufactured? 8. Do your products meet filtration and protection standards as recommended by OSHA, NIOSH and AORN? 9. What levels of protection are available for your products? Does your company have products available in 0.1- 0.3 microns filtration level? 10. Are your products designed to contain and filter droplets of microorganisms expelled from the mouth and nasopharynx during talking, sneezing, and coughing? 11. What is the average length of time your products can be worn while still maintaining the optimum level of protection? 12. Are your products made of material that is liquid-resistant and have low lint content? 13. Does your company offer products with a solid shield (wrap around guard) for the entire face (eye, nose, mouth)? 14. Does your company offer products that are low fog for eyeglass wearers? 15. What types of educational tools or materials do you have for this product line? 16. What kind of staff training does your company provide? 17. Does your company provide customer service 24/7? 18.
