This is a REVISED combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice TO ORIGINAL NOTICE W81XWH06T0002. CLOSE DATE FOR W81XWH 06T0002 HAS BEEN EXTENDED TO 19 OCT 05 NOON EST. EMAIL RESPONSES TO micaela.bowers1@us.army.mil. NAICS HAS BEEN CHANGED TO 334220. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be is sued. PLEASE NOTE REVISED STATEMENT OF OBJECTIVES: Statement of Objective (SOO) for Non-Complex, Low-Dollar Threshold Commercial Service Telemedicine and Advanced Technology Research Center (TATRC) Statement of Objectives Ad Hoc Bandwidth for TATRCs V-Sat Terminals Revision v.2 5 October 2005 1.0 Objectives 1.1 The Contractor shall provide Ad Hoc Bandwidth: 2.4MHz [768 Kbps duplex] as follows: 1.1.1 Base Service To/From Amount of Airtime Time Period CLIN 0001 Ft Detrick, MD  Ft Irwin, CA Thirty consecutive days Sometime during the period of 1 Nov 2005 to 30 Sep 2006 1.1.2 Option One  1 November to 31 December 2005 CLIN 0002 Ft Detrick, MD - Baltimore, MD Seven consecutive days During the period 1 Nov to 31 Dec 2005 1.1.3 Option Two  1 January to 31 December 2006. The Contractor shall provide Ad Hoc Bandwidth: 2.4MHz [768 Kbps duplex] as follows: CLIN 0001 Ft Detrick, MD to CONUS location to de determined Seven consecutive days CLIN 0002 Ft Detrick, MD to CONUS location to de determined Seven consecutive days CLIN 0003 Ft Detrick, MD to CONUS location to de determined Thirty consecutive days CLIN 0004 Ft Detrick, MD to CONUS location to de determined Thirty consecutive days 1.1.4 Option Three  1 January to 31 December 2007. The Contractor shall provide Ad Hoc Bandwidth: 2.4MHz [768 Kbps duplex] as follows: CLIN 0001 Ft Detrick, MD to CONUS location to de determined Seven consecutive days CLIN 0002 Ft Detrick, MD to CONUS location to de determined Seven consecutive days CLIN 0003 Ft Detrick, MD to CONUS location to de determined Thirty consecutive days CLIN 0004 Ft Detrick, MD to CONUS location to de determined Thirty consecutive days 1.2 Once specific training dates and locations are known, the Contractor will provide bandwidth within 72 hours of notification by TATRC. 1.3 The Contractor must provide service via the PanAmSat satellite system. 1.4 24/7 Help Desk Support for establishing satellite link and troubleshooting service is required. 2.0 Service Level Monitoring 2.1 Periodic and random monitoring of service will be performed by the COR in accordance with the approved Contractors Quality Assurance Plan. 3.0 Manpower Reporting 3.1 Deliverable. Accounting for Contract Services - Contractor Manpower Reporting. The contractor will comply with the following: The Office of the Assistant Secretary of the Army (Manpower Reserve Affairs) operates and maintains a secure Army data collection site where the manpower (including sub-contractor manpower) required for performance of this contract. The contractor is requi red to completely fill in all the information in the format using the following web address: https://contractormanpower.army.pentagon.mi1. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical Repre sentative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated d irect labor hours (including sub-contractors); (6) Estimated direct labor dollars paid this reporting period (including sub-contractors); (7) Total payments (including sub-contractors); (8) Predominant Federal Service Code reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if diff erent); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of r eporting this information); (11) Locations where contractor and (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contin gency contract language; and (13) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply wit h this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending 30 September of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may sub-contractors perform th e work use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a contractor's system to the secure web site without the need for separate data entries for e ach required data element at the web site. The specific formats for the XML direct transfer may be downloaded from the web site.
