Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, a written quote is being requested and a written solicitation will not be issued. This Request for Quatation (RFQ) Number 236-06-Q-0001 from the Indian Health Service in Rockville, Md. is for a Simplified Acquisition is for the purchase of packageing services and mass mailing of letters from the Rockville, Md. area to locations around the United States.Request for Quotation 236-06-Q-0001. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. This is a 100% Small Business Set-Aside. IHS Scholarship package assembly and mass mailing project scope of work. PURPOSE: The Indian Health Service is requesting quotes for the mass mailing service which includes the assembly of scholarship material, stuffing packet envelopes, labeling, mailing and tracking service. The mass mailing service is necessary to ensure that IHS Scholarship applications reach Indian students and schools for FY 2006 timely. The successful contractor?s mailing service shall be completed after receipt of all package material. Propsoals are due by COB, Oct. 21, 2005 (5:00 PM local time) SCOPE OF WORK: The mass mailing service will include the assembly/collation, stuffing, sealing, labeling, mailing (individual & bulk mailing) and mail tracking service of approximately 30,000 Scholarship application packets which weighs approximately 8.5 oz. Offers submitted shall include all cost i.e., labor, supplies, transportation, etc. necessary to perform the mass mailing service: 1. Assemble/Collating ? Scholarship packets will require assemble/collating of the following material into a 12? envelope package: a. Area Office Scholarship Coordinator?s cover letter - (a letter from each of 13 Area Coordinators) b. IHS HQE Scholarship Coordinator?s cover letter c. Application Instruction Book ? 8 ?? x 11? with thickness of 1/8?. d. Application for Participation ? (Purple bubble booklet) ? 4 pages (8 back to back) e. Return envelopes shall be blank, size 9 1/2? x 12? and folded 2. Labeling envelopes a. IHS will provide an Excel address document identifying the names and addresses of individual and organizations to receiving individual or bulk mailing packets. b. The Excel address document will also identify which individual/organizational recipient will require First or Fourth class mailing. c. The Excel sheet will identify number of bundles packets required. d. The contractor shall ensure that all address labels correctly reflect the address provided in the Excel address document. 3. Tracking system ? The contractor shall provide a tracking system to document mailing dates, addresses, delivery date, receipt information and utilize in verifying if returned packets were due to contractor address typing error or IHS incorrect addresses error. REPORTING REQUIREMENTS: The contractor shall provide the following reports: DESCRIBE REPORT TITLE, CONTENTS OF REPORT AND FORMAT. The report should include the following categories Number of individual packets mailed Number of Organizational packets mailed Number of packets mailed to Area Coordinators Number of undelivered scholarship packets Number of undelivered scholarship packets due to labeling error verified through tracking system Firms ahving the ability to provide the services listed above shall provide clear and comprehensive information supporting its claim, inclusive of references listing contact person(s) and phone number(s),technical information, and pricing.
