Description

This is a synopsis for a commercial acquisition using simplified acquisition procedures (Reference FAR Parts 12 and 13). THIS IS A SYNOPSIS AS REQUIRED BY FAR PART 5.2 ? THIS IS NOT A REQUEST FOR QUOTE/PROPOSAL. VENDORS SHOULD NOT SUBMIT FORMAL QUOTES/PROPOSALS UNTIL THE RFQ/RFP IS POSTED TO THIS WEBSITE. 45 CONS/LGCB has a requirement for one Non-Mydriatic Retinal Camera. Manufactured by Topcon Corp, model TRC-NW6S (Brand Name or equal.) This equipment is capable of taking panoramic images of the retina including the peripheral area and is used to screen and diagnose glaucoma and diabetic tetinopathy, two of the leading causes of blindness. Delivery and Installation must be included in quote. This service will be conducted at Patrick AFB, FL. Period of performance/expected delivery date will be contained within the solicitation package. The government intends to make a single award to the lowest priced, technically acceptable offeror. This proposed procurement is not a small-business set-aside. Solicitation No. FA2521-06-T-0006 will be issued as a Request for Proposal. It is advertised with a Standard Industrial Code (SIC) of 5048, business size of 500 employees and a North American Industry Classification System (NAICS) code of 423460. GSA Schedule holders for this item/service are encouraged to provide us with their GSA Schedule number. All responsible sources may submit a proposal, which shall be considered by the agency. The solicitation will be issued electronically on or about 19 Oct 05 at www.eps.gov . Proposals may be submitted via hard copy, fax (321-494-2706), or e-mail. NO HARD COPIES OF THE SOLICITATION WILL BE ISSUED FOR ANY REASON. To be eligible for award, prospective offerors must be registered in Central Contractor Registration (CCR) at the CCR Internet site, http://www.ccr.gov.
