Description

J-Maintenance, Repair, and Rebuilding or Equipment The Sierra Pacific Network Consolidated Activity (CCA) is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 to provide Preventive Maintenance and Repair Services for Ohmeda anesthesia machines located at the VA Sierra Nevada Health Care System, 1000 Locust Street, Reno, NV. Contractor shall provide all necessary supervision, parts, labor, travel, materials and equipment to perform quarterly preventative maintenance and necessary repair service calls in accordance with manufacturers? recommendations on Ohmeda anesthesia machines for the period of December 1, 2005, or date of award if subsequent thereto, through September 30, 2006 with four one-year options through September 30, 2010. Contract award is subject to availability of funds. The following provisions and clauses apply in addition to any addenda: FAR 52.212-1 Instructions to Offerors ? Commercial Items; 52.212-2 Evaluation ? Commercial Items; 52.212-3 Offeror Representations and Certifications ? Commercial Items; 52.212-4 Contract Terms and Conditions ?
