- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W9124D-06-S-SURG is issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisiti on Circular 01-25. This is an unrestricted solicitation. Applicable NAICS code is 339113. See attachment List of Supplies for items that need to be priced. All items are KLS Martin or equal. Orthopedic Supplies for Chin and Jaw Surgeries on an as need ed basis for the period 1 Nov 2005 through 30 Sep 2006, FOB Destination. Award will be made to the lowest bidder on an all or none basis and whose prices are determined to be fair and reasonable.
