Description
This is a COMBINED SYNOPSIS/SOLICITATION FOR COMMERICAL ITEMS PREPARED IN ACCORDANCE WITH THE INFORMATION IN far Subpart 12.6, using Simplified Acquisition Procedures for commercial items found at FAR 13, as supplemented with the additional information included in this notice. Naval Medical Center San Diego (NMCSD) intends to solicit and negotiate on a sole source basis with Mateson Tri-gas in accordance with FAR 6.302-1. NMCSD intends to award a firm fixed price type contract. This announcement constitutes the only solicitation; a written solicitation will not be issued PAPER COPIES OF THIS SOLICTATION WILL NOT BE AVAILABLE. The RFQ number is N00259-05-T- 0009. This solicitation documents and incorporated provisions and clauses in effect through FAC 2005-06. It is the contractor?s responsibility to be familiar with the applicable clauses and provisions. Description of supplies and services is as follows: CLIN 0001 Rental of Liquid Oxygen Tank for the following sizes: Main tank 10,000 Gal. capacity, Reserve tank 650 Gal. capacity. Rental service period 29 Oct 05 to 30 Apr 06. QTY: 5 MO x ________=___________. CLIN 0001: _____________ CLIN 0002 Delivery of Liquid Oxygen to maintain the Capacity of main tank 10,000 Gal. Capacity of reserve tank 650 Gal. Deliveries for the period of 29 Oct 05 to 30 Apr 06. QTY: 50,000 Gallons x ________=_______________. CLIN 0002: ______________ Offeror shall be responsible for delivery of Liquid Oxygen Bi-weekly in accordance with the following schedule: 04 Nov 05, 18 Nov 05, 02 Dec 05, 16 Dec 05, 30 Dec 05, 13 Jan 06, 27 Jan 06, 10 Feb 06, 24 Feb 06, 10 Mar 06, 17 Mar 06, 31 Mar 06, 14 Apr 06, 28 Apr 06. Period Of Performance is 29 October 2005 Thru 30 April 2006. Responsibility and Inspection: unless otherwise specified in the order, the supplier is responsible for the performance of all inspection requirements and quality controls. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals. However, all proposals received with (4 days )after date of publication of this synopsis will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Parties responding to this solicitation may submit their quote in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.) but must include company?s complete mailing and remittance addresses, discounts for prompt payment, if any, anticipated delivery/availability of products, company?s CAGE code, Dun & Bradstreet Number, and Taxpayer ID number. Destination. OFFERORS ARE REQUIRED TO COMPLETE AND INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPOSALS: FAR 52.212-3, Offeror Representation and Certifications ? Commercial Items. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) past performance. Provide references, specifically addressing: laboratory reagent supplies in the last 3 years. (2) technical capability. Defined as an assessment of the prospective contractor's key management personnel to determine if they have the basic knowledge, experience, and understanding of the requirements necessary to produce the required product or provide the required service and (3) price. Contractors who do not meet the first 2 factors will not undergo a price evaluation. Past Performance and Technical Capability will have more importance than cost or price.
