NOTICE. This position is set-aside for an individual Vascular Technologist. Applications from companies will not be considered; additionally, applications from active duty Navy personnel, civilian employees of the Navy, or persons currently performing medical services under other Navy contracts will not be considered without the prior approval of the Contracting Officer. B. POSITION SYNOPSIS. The Government is seeking to place under contract, an individual who holds a current, unrestricted license to practice medicine in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands and is Board Certified in Radiology. This individual must also (1) meet all the requirements contained herein; and (2) competitively win this contract award. You shall serve as a Vascular Technologist, providing care and services to inpatients and outpatients for the Vascular Surgery division in the General Surgery department located in the Naval Medical Center Portsmouth, VA. You shall normally provide services Monday through Friday between the hours of 0645 to 1800 (6:45am to 6:00pm). The healthcare workers’ specific duty hours will be scheduled by the Director of Vascular Laboratory or his/her authorized representative and will vary based upon workload. The healthcare workers’ shifts shall not exceed 10 hours and will include an uncompensated 30-minute meal break. The healthcare workers shall not exceed 80 hours in a two-week period. The healthcare worker will be responsible for tracking hours. You shall not be required to provide services on the following 10 Federal holidays: New Year’s Day, Martin Luther King’s Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day. When required, to ensure completion of services that extend beyond the normal close of business, you shall remain on duty in excess of the scheduled shift. You shall be given an equal amount of compensatory time to be scheduled upon mutual agreement of you and the Commanding Officer. You shall arrive for each scheduled shift in a well-rested condition and shall have had at least six hours of rest from all other duties as a Vascular Technologist. You shall accrue 10 hours of paid leave (i.e. vacation and sick leave) per 80 hours worked or in an approved leave status. You shall be fully compensated for these periods of authorized leave. Any leave accrued by you can be carried over through 31 January 2006. Any leave not used by 31 January 2006 will be forfeited. This position is for a period beginning from the start date (a date agreed upon by the successful applicant and the Government) through 31 January 2006. In the event of Government delays (late award of order, extended process time of new employee etc) the Government reserves the right to extend contract to provide services up to the limit of hours listed within Section B. II. STATEMENT OF WORK A. The use of “Commander” means Commanding Officer, Naval Medical Center, Portsmouth, VA, or designated representative, e.g., Technical Liaison, Department Head, or his/her authorized representative. B. THE VASCULAR TECHNOLOGIST AS A PERSONAL SERVANT. The services you are providing under this contract shall be rendered as personal services for the Navy, performing an agency function by providing direct medical care required by the Navy for its health care beneficiaries. Prior to performance of services, you shall submit to the technical liaison credentialing documents as required. Your activities shall be subject to day-to-day supervision and control by Navy personnel in a manner comparable to the supervision and control exercised over Navy uniformed and civil service personnel engaged in comparable work. The term “supervision and control” is defined as that process by which you receive technical guidance, direction and approval with regard to an element of work or a series of tasks within the requirements of this contract. It is the intent of the parties that these personal service contracts create an employer-employee relationship between you and the Navy. Accordingly, personal injury claims alleging negligence by you within the scope of your performance of this contract shall be processed as claims alleging negligence by DOD military or civil service employees. You are not required to maintain medical liability insurance. C. DUTIES AND RESPONSIBILITIES. You shall perform a full range of Vascular Technologist services for inpatients and outpatients in support of the Vascular Division, Department of General Surgery, using government furnished supplies, facilities and equipment within the assigned unit of the Medical Treatment Facility (MTF). Your productivity is expected to be comparable with that of other individuals performing similar services. You shall perform the following duties: 1. Perform direct noninvasive vascular diagnostic studies such as carotid duplex, upper and lower arterial and venous duplex, renal artery, SMA, celiac artery and portal hepatic duplex. 2. Perform physiologic noninvasive vascular diagnostic studies such as upper and lower extremity arterial pulse volume recording (PVR), segmental pressures, continuous wave Doppler, arterial and venous photoplethysmography (PPG) and treadmill exercise testing. 3. Operate noninvasive vascular diagnostic equipment such as duplex imager, treadmill, PVR, continuous wave Doppler and PPG. 4. Educate patients with peripheral vascular disease to promote health and avoid complication of their disease process. 5. Assist the vascular nurses in the triage, scheduling and follow-up examinations of patients in the Vascular Surgery division, as well as, patients referred for diagnostic studies from other divisions, outpatient clinics and inpatients of the NMCP. 6. Provide training for the corps staff of the Department of Surgery and other hospital departments in the use of basic noninvasive vascular equipment. 7. Participate in research projects of the Division of Vascular Surgery to improve the diagnosis and care of the patients with vascular disease. 8. Maintain the diagnostic equipment of the Noninvasive Vascular Laboratory, ensuring appropriate maintenance scheduling and ordering of consumable supplies. 9. Participate in the ongoing quality assurance program of the Vascular Laboratory to maintain standards of accuracy. 10. Demonstrate knowledge in all aspects of noninvasive vascular testing: test purpose and definition, anatomy and physiology, pathology involved, test procedure, instrumentation and test interpretation and communicate all findings both orally and in writing to health care providers. 11. Demonstrate knowledge of Doppler and vascular physics and hemodynamics of arterial and venous systems in normal and abnormal patients. 12. Demonstrate knowledge of physics and hemodynamics in the development and acquisition of new diagnostic technology. 13. Demonstrate knowledge of computerized data retrieval systems for patient scheduling, organization of laboratory results and quality assurance programs. 14. Maintain a supply inventory, arrange for appropriate and regularly scheduled maintenance of complex diagnostic equipment and arrange for repair or replacement of broken vascular laboratory equipment. 15. Orient physicians, nurses, corps staff and students in the health care disciplines to the scope of diagnostic procedures, treatment, and health care interventions for patients with vascular disease. 16. Participate in the evaluation of diagnostic equipment and supplies used in the care of patients with vascular disease. 17. Demonstrate awareness and sensitivity to patient/significant others' rights, as identified within the institution. 18. Demonstrate awareness of legal issues in all aspects of patient care and unit function and strive to manage situations in a reduced risk manner. 19. Participate in peer review and performance improvement activities. 20. Demonstrate appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities. 21. Maintain an awareness of responsibility and accountability for own professional practice. 22. Participate in continuing education to meet own professional growth. 23. Participate in the provision of in-service training to clinic staff members. 24. Attend annual renewal of hospital provided annual training such as family advocacy, disaster training, infection control, sexual harassment, and bloodborne pathogens. 25. Participate in monthly meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. Should a meeting occur outside of your regular working hours, you shall be required to read and initial the minutes of the meeting. 26. CRIME CONTROL ACT OF 1990 REQUIREMENT. 26.1. Section 21 of the Crime Control Act of 1990, 42 U.S.C. 13041, as amended by section 1094 of Public Law 102-190, requires every facility operated by the Federal Government (or operated under contract with the Federal Government) that hires (or contracts to hire) individuals involved in the provision of child care services to assure that all existing and newly-hired employees undergo a criminal background check. The term "child care services" is defined to include health and mental health care. 26.2. The Government will conduct criminal background checks on all contractor employees providing child care services under this contract based on fingerprints of contractor employees obtained by a Government law enforcement officer and inquiries conducted through the Federal Bureau of Investigation (FBI) and State criminal history repositories. 26.3. Within 30 days after contract award, the contractor and all contractor employees shall provide all reasonable and necessary assistance to the Government, including compliance with the employment application requirements set forth in 42 U.S.C. 13041(d). Upon receipt of the results of a background check, the contractor further agrees to undertake a personnel action in accordance with 42 U.S.C. 13041(c), when appropriate. 26.4. With written recommendation from the Commanding Officer and the approval one level above the Contracting Officer, a contractor employee may be permitted to perform work under this contract prior to the completion of a background check, provided the employee is within sight and continuous supervision of an individual with a successful background check. 26.5. Contractor employees shall have the right to obtain a copy of any background check pertaining to themselves and to challenge the accuracy and completeness of the information contained in the report. 27. HIPPA. The HCW shall comply with the HIPPA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facilities. 28. If not a Registered Vascular Technician, the HCW shall obtain registry within the first 12-14 months of contract start. D. MINIMUM PERSONNEL QUALIFICATIONS. To be qualified for this position you must: 1. Certification as a Registered Vascular Technician (RVT) by the American Registry for Diagnostic Medical Sonography (ARDMS); or registry eligible with application in the current year; or graduation from a training program in either vascular technology or cardiovascular technology that has been accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Canadian Medical Association (CMA). 2. A minimum of 12 months full time (at least 35 hours/week) clinical vascular testing experience within the last 36 months. In addition, you shall have one of the following: a. Completion of a formal two-year program or equivalent in another allied health profession; or, b. Completion of a bachelor’s degree unrelated to vascular technology; or, c. At least 600 noninvasive vascular examinations under the supervision of medical and technical staff that meet the above criteria. The noninvasive vascular examinations performed by these technical staff members must be appropriately distributed among the testing areas within the laboratory. 3. Possess current certification in Basic Life Support (BLS). 4. Provide two letters of recommendation written within the last two years attesting to your clinical skills. A minimum of one of the letters must be from a physician supervisor. Reference letters must include name, title, phone number, date of reference, address and signature of the individual providing reference. 5. Be eligible for U.S. employment. 6. Represent an acceptable malpractice risk to the Navy. 7. Submit a fair and reasonable price, which has been accepted by the Government. E. FACTORS TO BE USED IN A CONTRACT AWARD DECISION. If you meet the minimum qualifications listed in the paragraph above entitled "Minimum Personnel Qualifications" you will be ranked against all other qualified candidates using the following criteria, listed in descending order of importance. The "Personal Qualification Sheet," Letters of Recommendation and the DD 214 (if you possess prior military services) shall be used to evaluate these items. 1. Applicants who possess a Certification as a Registered Vascular Technologist (RVT), then 2. Quantity and quality of experience to perform the duties as specified herein, then 3. Letters of recommendation required in item D.4, above, may enhance your ranking if they address such items as clinical skills, professionalism, or specific areas of expertise, etc., then 4. Total Continuing Medical Education (CME) hours in the last 2 years, then 5. Experience as a vascular technologist in a DOD medical facility. F GENERAL REQUIREMENTS FOR ALL HCWs. 1.1.1. PROOF OF U.S. EMPLOYMENT ELIGIBILITY No alien shall provide services under this contract in violation of the Immigration Laws of the United States. 1.1.2 REPRESENT AN ACCEPTABLE MALPRACTICE RISK TO THE DEPARTMENT OF DEFENSE. Health care providers and practitioners shall submit a signed, detailed statement of prior or pending malpractice claims and an explanation of disposition of each claim. 1.1.4 LANGUAGE SKILLS. HCWs shall read, write, speak, and understand the English language with sufficient fluency to maintain effective communication skills with patients, staff and other members of the healthcare industry. 1.1.5 BLS/CPR CERTIFICATION. Maintain certification in American Heart Association Basic Life Support (BLS) for Healthcare Providers; American Heart Association Healthcare Provider Course; American Red Cross CPR (Cardio Pulmonary Resuscitation) for the Professional Rescuer; or, equivalent. In the event an otherwise qualified HCW does not possess this certification and the MTF elects to provide it, the Government reserves the right to deduct 4 hours of compensated service. This deduction shall apply to initial certification only; consideration will be based upon the specific HCW's CLIN/SLIN. The Government may provide recertification. 1.1.6 COMPUTER SKILLS COMPETENCY. The Contractor shall submit proof of Computer Skills Competency prior to each healthcare worker providing services under each Task Order. The Contractor shall utilize the Computer Skills Competency Form (Government to supply as required). 1.1.7 PERSONAL AND PROFESSIONAL INFORMATION SHEET (PPIS). Complete and submit a PPIS (per BUMEDINST 6320.66D). This will be provided with requirement specific TO. 1.1.8 LATEX ALLERGY SCREENING QUESTIONNAIRE. Complete and submit the Latex Allergy Screening Questionnaire. 1.1.9 REGULATORY COMPLIANCE REQUIREMENTS. 1.1.14 PHYSICAL EXAMINATION. The HCW shall obtain, at Contractor expense, a statement from the HCW's physician or a report of a physical examination within 60 days prior to the date of the Task Order. This statement or report shall indicate whether or not the HCW is free from mental or physical impairments that would restrict them from providing the services described herein. HCWs hired after Task Order start shall provide a physical examination statement or report obtained within 60 days prior to performance of services. Expenses for all required tests and/or procedures shall be borne by the Contractor at no expense to the Government. Statements from the HCW's physician shall contain the following information: "Having performed a physical examination on the person named below on (insert date), the following findings are true and accurate: 1. (Name of HCW) is suffering from no physical disability that restricts them from providing services as a (HCW’s specialty). 2. (Name of HCW) is not suffering from sexually transmitted or other contagious diseases, which restricts them from providing services as a (HCW’s specialty).
