Date: November 4, 2005 Closing Date: November 10, 2005 4:30 PM EST RFP: 541-020-06 From: Louis Stokes Cleveland, Department of Veterans Affairs 10000 Brecksville Road, Brecksville, OH 44141 For Information call: Richard Dahmen, Contracting Officer Tel: 440-717-2818 Email: Richard.dahmen@med.va.gov The Louis Stokes Cleveland VA Medical Center, Cleveland Ohio is in need of a Vendor to perform decontamination cleaning at the VA Medical Center, 10701 East Boulevard, Cleveland, Ohio, and 10000 Brecksville Rd., Brecksville, Ohio. This contract shall be set-aside to small business relative to NAICS code 561720. The term of the contract shall be Date of award through September 30, 2010. The contract shall be a base year and four options years totaling 5 years. Award will be made using Best Value considerations, with technical and past performance greater than price. This RFQ states that technical merit shall be considered significantly more important than cost/price and that the contract may be issued to other than the vendor submitting the lowest priced, technically acceptable quotation. Award will be made based on availability of one vendor. Agency submission to VA: Agency shall provide the following, to be reviewed for the Best Value Selection: 1. Technical CAPABILITY: Offerors shall provide evidence of general capabilities, key personnel (Project manager and off-site representative, if different individuals). a. Knowledge of computer room and telephone switch room cleaning b. Insurance Certification c. Resume d. Familiar with existence equipment e. Knowledge in Class 10 clean rooms 2. Past Performance: The Offeror shall provide a listing of all contracts performed within the past three (3) years that are similar in nature and relevant to the efforts required by this solicitation. The Offeror shall furnish the following information: a. Customer (Government or Commercial) and point of contact to include name, address, telephone number, fax number, and email address for each contact. 3. Price: Price offer, to include Hourly, Overtime, and any additional costs that may be incurred. Total cumulative price will be a factor in the decision making, not just per hourly rate. PERIOD YEAR PRICE PER YEAR Base Year: Date of award through September 30, 2006 __________________ Option Year 1: October 1, 2006 through September 30, 2007 __________________ Option Year 2: October 1, 2007 through September 30, 2008 __________________ Option Year 3: October 1, 2008 through September 30, 2009 __________________ Option Year 4: October 1, 2009 through September 30, 2010 __________________ Statement of Work: ANNUAL (1 QTR.) SUB-FLOOR CLEANING: Top of Tile Surface Preparation for Sub-Floor Cleaning Top of Tile Preparation: -Vacuum top of all tiled surfaces to include underneath hardware where accessible, ramp areas where applicable. -Tacky roller top of all tiled areas. -Using specially treated anti-static cloths, we will dust under hardware and all other accessible areas. The above methods will remove all loose dirt, dust and debris from the top surface of the raised access floor. This methodology insures that all contaminants are removed and not directed from one area to another. Sub-Floor Cleaning Sub-Floor Cleaning: The Vendor maintains the structural integrity of the raised access flooring during their cleaning process. Our service technicians shall remove only one tile at a time except when necessary to access a difficult to reach area. -Vacuum under all sub-flooring area to include walls, floors, vents, pipes and conduit etc. -Dust under flooring with low particulate generating wipes. -Special attention given to stringers using a two-step method, vacuuming and wiping with low particulate generating wipes. -Remove any debris under flooring. -Clean perforated airflow panels. -Level floor tiles as necessary. Hardware & Flat Surface Cleaning -Clean exterior of all hardware cabinets using a non-ionic cleaner, preventing static charge. -Clean interior of all network cabinets. -Clean screens. -Clean keyboards and monitors using lint-free dusting cloths and non-ionic cleaning solutions. -Clean all flat surfaces within arm's reach. -Clean all interior windows. Top of Tile Cleaning -Remove any scuffs and/or stains. -Clean top of all tiled areas using a non-ionic (non-static producing) solution. -Detailed cleaning at entrance points (ramp and handrails, etc). Complete Sub-Floor Inspection -Environmental inspection -Raised access floor structural inspection QUARTERLY (3 QTRS.) ENVIRONMENTAL MAINTENANCE CLEANINGS: Top of Tile Surface Preparation Top of Tile Preparation: Vacuum top of all tiled surfaces to include underneath hardware where accessible, ramp areas where applicable. -Tacky roller top of all tiled areas. -Using specially treated anti-static cloths, we will dust under hardware and all other accessible areas. Computer rooms located in Bldg 6 rooms 31 and 24 at the Brecksville facility. The telephone switch rooms are room A108 at the Brecksville facility and room 3415 at the Wade Park facility.
