- This RFP/SOLICITATION WILL BE RELEASE ON FedBizOps 8/3/2005 AS FOLLOWS: REQUEST FOR PROPOSAL (RFP)Number: DOL059RP20078 You are invited to submit a separate business/cost and technical proposal to the U.S. Department of Labor (DOL) for services related to Technical Support for the purpose of this SOW is to procure assistance for the OASAM Office of Worker Safety and Health in the creation, management and oversight of the DOL workplace safety, occupational health, and workers’ compensation programs and assist with the day-to-day operations of the office. Towards this end the contractor shall provide technical support required to develop, implement and evaluate DOL internal programs and provide accurate assistance and advice to agency and regional safety, occupational health, and workers’ compensation staff in accordance with the Occupational Safety and Health Act of 1970, Executive Order 12196, 29 CFR 1960, Federal Employees Compensation Act (FECA), related laws, policies, regulations and standards, and DOL policy requirements. These technical support tasks will implement DOL’s internal policies, plans, practices, and procedures in support of the Department’s workplace safety, occupational health, and workers’ compensation goals and mission. Duties may involve associated administrative functions associated with workplace safety, occupational health, and workers’ compensation programs oversight, administration and tracking for four years, Base and three (3) Option Years as described in this RFP and the enclosed STATEMENT OF WORK (SOW). This requirement is in support of the OASAM, Office of Safety and Health Services. This requirement is for competition, SET-ASIDE FOR SERVICE DISABLE VETERAN OWNED-SMALL BUSINESS. Your Service Disable Veteran Small Business company is receiving this RFP and Performance Base, Statement of Work (SOW) requirement as a result of the U.S. Department of Small Business Administration (SBA), 8(a) Program- 8 (a) Competition. Please provide firm fixed price for this effort for this requirement using the enclosed Section B. Based on market research, this procurement is under NAICS CODE 541990. Please provide firm fixed price, labor hour rate for this effort to be utilized in placing task orders for this requirement. For labor categories, please used the enclosed (Section B.2.). PAST PERFORMANCE. Offerors shall provide the names of at least three (3) business references that may be contacted by this office for relevant contracts and sub-contracts within the last three (3) years in the technical proposal. The reference should be on project/work similar in nature.You may list federal government, agencies of state and local governments, and commercial customer sources. Include the name of the contact person and a telephone number for the preferences you provide as follows: 1. Name of contracting activity; 2. Contract and/or BPA Agreement number; 3. Task order numbers; 4. Contract type; 5. Total contract value; 6. Contract work; 7. Contracting officer and telephone; 8. Program manager and telephone; 9. Contracting officer technical representative and telephone ; 10. Administrative contracting officer, if different from #9 and telephone number; and 11. List of major subcontractors. Government reserves the rights to assess other past performance data. Your separate business/cost proposal (original copy and three (3) photocopies) and separate technical proposal (original copy and four (4) photocopies) must be received by the Office of Procurement Services (OPS), U.S. Department of Labor (DOL), 200 Constitution Ave., NW, Room N-5416, Washington, DC, 20210, NOT LATER THAN August 15, 2005, 1:30PM, Washington, DC E.S.T.. No other compensation will be paid to the contractor. SUBMISSION OF QUESTIONS CONCERNING THIS RFP and STATEMENT OF WORK: The cutoff date for receipt of all questions regarding this RFP or the attached SOW shall be submitted via e-mail NOT LATER THAN August 8, 4:30PM, Washington, DC, E.S.T. to friloux.allison@dol.gov. Do not submit questions to any other DOL employee source, unless otherwise directed by the contracting officer noted below in this document. The offeror shall include, with each separate business/cost and separate technical proposal, a cover letter, must be signed by an official authorized to bind the offeror, and it shall contain a statement to the effect that the proposal is firm for a period of at least one hundred and twenty (120) days from the date of receipt by the Government. The RFP does not commit the Government to pay any costs incurred in the submission of your proposals or to contract for the articles or services. It is also brought to your attention that the Contracting Officer is the only individual who can commit the Government to the expenditures of public funds in connection with this procurement.
