Description

This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedure in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The RFQ number is 672-430-06 and should be reference on all correspondence. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-06. The NAICS is 423450 and the size standard is $ 100 millions. Offerors? proposals are hereby requested in response to the requirement under a firm-fixed price contract and shall be submitted on offerors? letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Offerors shall submit all technical questions regarding the solicitation to the Contracting Officer, in writing, no later than three (3) calendar days after issuance of this Request for Quotation (RFQ). Questions will not be accepted after November 28, 2005. Questions may be sent via e-mail at Minerva.Alonso@med.va.gov. Verbal questions will not be addressed. Quotes are to be provided for the additives to be installed on the HILLROM CAREASSIST BEDS located at our facility. The items are as follows: CLIN 001- OSI Care Assist Single Care Off Set Frame - Item#3255 Qty. 25 ea price $______________ CLIN 002 ? Patient Helper Bracket - Item#3429-1 Qty. 25 ea price $ ____________ CLIN 003 ? Foot End Fracture Frame Bracket ? Item# 3228 Qty. 25 ea price $ ___________ The quote is to include delivery terms as FOB Destination (Door to Door) and be valid for thirty days from the offer due date. All responses must include the following: (1) descriptive literature and product numbers for the items offered providing the products offered meet the above minimum requirements; (2) warranty information. Award will be made to the lowest offeror whose offer meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers are to be submitted on company letterhead and include pricing information, the information required in provisions 52.212-1, a completed copy of the provision of 52.212-3 Offeror Representations and Certifications-Commercial Items (Mar 2005) and the company DUNS number. All offerors are responsible for any amendment done to this solicitation on or before the solicitation due date. Offerors shall visit the BOS web site www.bos.oamm.va.gov in order to download such amendments in case is necessary. Facsimile proposals shall be accepted at 787-781-8721.
