Description
SAMPLE COMBINED SYNOPSIS SOLICITATION IAW FAR 12.603 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement number F3ZT885319A100, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-06 and Defense DCN 20051114. The solicitation/acquisition listed here is unrestricted. The Federal Supply Class (FSC) is 5805 The associated Standard Industrial Classification (SIC) is 4812. The North American Industry Classification System is 517212 and the associated business size standard is 1,500 Employees . Description of Requirement: Both analog and digital cell phone (wireless) service for 91 phones/squadron personnel at various northern California locations to be effective 16 Dec 05. Important features include: Tri-mode (analog and digital) capability for enhanced coverage of Northern California mountainous and outlying areas is extremely important Roughly 61,200 monthly pooled anytime minutes for the 91 phones; average of roughly 700 anytime minutes per phone/per month Unlimited mobile to mobile Free nights and weekends Free roaming Free long distance Free phones, battery, and wall charger Portability of current cell phone numbers Free activation and mailing of cell phones to approximately 50 Northern California office locations Tax exemption Performance Periods (two): Basic: 16 Dec 05 thru 30 Sep 06 (submit the price per phone per month); and Option: 1 Oct 06 thru 30 Sep 07. The following provisions and or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Commercial. Offerors must meet 15 Dec 05 delivery date for new phones and confirm that analog coverage is available. FAR 52.212-2 Evaluation-Commercial Items, paragraph (a) to this provision is completed as follows: the best value offer that meets the requirements. FAR 52.212-3, Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision will not be considered for award unless the offeror holds a Federal Supply Schedule Contract. Contractor must ensure annual reps and certs are current or complete them at http://www.bpn.gov/orca/. FAR 52.217-8 Option to Extend the Term of the Contract, and 52.232-18, Availability of Funds (Applicable to the option period). The following clauses/provisions will be included in open market orders. FAR 52.212-4, Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items include clauses (b) (16), (17), (18), (19), (20), and (c) N/A. DFAR 252.212-7001, Contract Terms and Conditions (Dev). DFARS 252.204-7004 Alt A, Required Central Contractor Registration. Payments may be made by Electronic Funds Transfer OR by Government Purchase Card; and the successful offeror will be notified of the payment method prior to award.
