- This is a synopsis for a commercial acquisition using simplified acquisition procedures (Reference FAR Parts 12 and 13). THIS IS A SYNOPSIS AS REQUIRED BY FAR PART 5.2 ? THIS IS NOT A REQUEST FOR QUOTE/PROPOSAL. VENDORS SHOULD NOT SUBMIT FORMAL QUOTES/PROPOSALS UNTIL THE RFQ/RFP IS POSTED TO THIS WEBSITE. 45 CONS/LGCBA has a requirement to perform two (2) preventive maintenance inspections per year, routine and emergency repairs on one General Electric Logiq 9 Ultrasound Unit, serial number 6391OUS2, equipment control number 5470, in accordance with statement of work (SOW). Accessories to be included: M12L (MIH) Probe; E8C Probe; 10L Probe; 3.5C Convex Probe; L9B-Flow Option; L9 Dicom Option; Sony UP51MD Color Printer; and 8300 PACS Link Connector. Performance period will be from 6 Jan 2006 ? 30 Sep 2006, with 4 (one) year options. This proposed procurement is a 100% small business set-aside. It is advertised with a Standard Industrial Code (SIC) of 7629, business size of $6M and a North American Industry Classification System (NAICS) code of 811219. All responsible sources may submit a proposal, which shall be considered by the agency. Solicitation No. FA2521-06-T-0014, Request for Quotation will be issued electronically on or about 29 Nov 2005 at www.eps.gov. Proposals may be submitted via hard copy, fax (321-494-2706), or e-mail. Proposals must be received not later than 29 Dec 2005, 3 P.M. NO HARD COPIES OF THE SOLICITATION WILL BE ISSUED FOR ANY REASON. Anticipated award date will be on or around 6 Jan 2006.
