This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; request f or quotations are being requested and a written solicitation will not be issued. Solicitation Number: W91QF6-06-T-0005 is issued as a request for quotation. Purchase of 8500 Bar Code Labels to be delivered 23 December 2005 to Fort Meade, MD Posted Date: 18 Nov 2005 Original Response Date: 12 December 2005 16:30 EST. Classification Code Set Aside: Total Small Business. The applicable NAICS code is 323119 with a size standard of 500 employees. There are contract lines (0001) to (0003) for a total quantity of 8500 ea. CLIN 0001Quantity: 5,000 Unit: Roll Supplies/Services: Military Bar Code Labels; CLIN0002 Quantity: 2,975 Unit: Roll, Supplies/Services: Civilian Bar Code Labels; CLIN0003 Quantity: 525 Unit: Roll, Supplies/Services: Civilian Bar Code Labels (Your price shall be FOB destination. Inspection and acceptance shall be at destination. Award shall be made to the best value, delivery, technical, past performance and price proposal. You shall submit commercially available literature on your unit that demonstrates all of the above capabilities are met. Description below: SUBJECT: Statement of Work for Purchase of Bar Coded Labels 1. Vendor must be able to provide custom labels in the following format: a. Design #1: M00-0000Y-00Y 1. M = Ft. Meade; 0000Y-00Y = the Y batch and the Y specimen in the batch (five digits:00001-001 = the 1st batch and the1st specimen in the batch, 912822-025 = the 91,822nd batch and the 25th specimen in the batch) 2. Batch number increments by one and specimen number increments from 1 through 100 3. Bar code message must be code 128 4. Each batch shall be completely contained on 1 roll of 100 specimens 5. Beginning of each roll shall be identified by the roll number, beginning sequence and ending sequence b. Bar code labels must be laminated c. There must be zero defects in bar code message d. The adhesive material operating range must be between -40 C to 121 C e. Label must remain attached to LSE (polypropylene) sample bottle at -40 C and 90% relative humidity for up to 2 years and to paper forms at room temperature/humidity for 5 years. In addition, label must be able to adhere to glass f. The labels shall be produced by Digitex, Photographic, or Digital Printing and laminated on a release liner g. Each set shall be 1.87 inches wide by 1.66 inches long h. Each label set shall consist of 1 bar code label with eye readable number (.525 inches x 1.66 inches), 2 bar code labels with eye readable (.3125 inches x 1.66 inches), and 1 bar code label with eye readable (.720 inches x 1.66 inches) i. Vendor shall be ISO9000 certified or equivalent j. The production yield, the percent delivered after quality control removed all bad labels must be 98% or greater ( all label sets removed must have scannable bar code label on outside of roll) k. Specimen number increments in 1. 4. Each roll shall have a maximum of 100 labels 5. Bar code message must be code 128 6. The beginning of each roll must be identified by the roll number, beginning sequence and ending sequence l. Bar code construction same as item #1(zero defects in label, laminated labels) m. Adhesive material characteristics same as item #1 n. Each roll of labels must be able to be mounted on a dispensa-matic label dispenser o. Quality control standards/measures same as item #1 p. Type of print same as item #1 q. Each set shall be 2.110 inches wide by 1.75 inches long r. Each label set shall consist of 1 bar code label with eye readable number (.630 inches x 175 inches), 2 bar code labels with eye readable (.380 inches x 1.75 inches), and 1 bar code label with eye readable (.720 inches x 1.75 inches) s. Design #3 1. MCA-00000-00Y and MCB-00000-00Y (alternating MCA and MCB label set; ODD number set MCA, EVEN number set MCB); 00000-00Y = the Y specimen( eight digits; 00000-001 = the 1st specimen, 00031-251 = the 31,251st specimen 2. MCA label set same as item #2; MCB label set shall consist of 1 bar code label with eye readable number (.630 inches x 1.75 inches), 2 bar code labels with eye readable (.380 inches x 1.75 inches), and 1 blank white label (.720 inches x 1.74 inches); B= fixed character (always B) 3.
