Description

<font color=purple size=3 type=”Century Gothic”>Wilkes Barre VAMC is seeking a contractor to provide a complete vendor-operated outpatient clinic for eligible veterans in the area within a 30-mile radius of Bloomsburg, PA. January 2005 enrollment was 1968; in October 2005, 2009 patients. Beneficiaries residing in the following counties are eligible for enrollment: Columbia, Montour, Northumberland, Union, Snyder, southwestern Lycoming, southern Sullivan, western Luzerne, and northern Schuylkill. Historically the CBOC contract has been priced on a per-member per month/ capitated basis. Contract period is date of award for one year with four (4) one year option renewals. The NAICS is 621111; size standard is $8.5M. Interested vendors should reply to the contracting officer via email titled “WBVAMC Columbia County CBOC – Interested Source” by COB Friday 23 December 2005 and address the following: 1. business name and DUNS number, 2. physical address(es) of clinic site(s) proposed, 3. industry certification(s) currently held by each site listed above, 4. a list of the ancillary services available on site, 5. business size certification, 6. that the firm’s www.ccr.gov registration is current and accurate, and 7. that the firm or any employee is not currently on the Excluded List at www.epls.gov . Responses will help determine whether the resulting solicitation will be set aside for small business or open to vendors of all sizes.
