Description
Solicitation for the possible replacement of Surgical Face Masks for TRICARE Puget Sound and Alaska. These products include but are not limited to a complete product line with a variety of sizes and types. Those companies interested in a possible R egional Incentive Agreement (discounted DAPA pricing based on volume) with TRICARE Puget Sound and Alaska must respond to the following questions: 1. Does your company carry a full product line of disposal hypoallergenic surgical face masks to include different sizes and styles? If so, please provide a list. 2. What are the standards by which your products are manufactured? Do you comply with NIOSH, OSHA, AORN and industry standards? 3. Does your company have a distribution agreement with Cardinal Healthcare? 4. Does your company currently possess, or in the process of obtaining, a Distribution and Pricing Agreement (DAPA)? 5. Does your company have the ability to service to the entire TRICARE Puget Sound and Alaska Region (WA and Alaska)? Please provide your companys distribution network 6. Can your company provide customer support (1-800-numbers and product representatives for each account)? 7. Does your company provide educational programs and clinical support? Describe material available and type of clinical support (brochures, video tapes, interactive access to web or other). Will product in-service and education be available upon request after product implementation throughout the length of a possible agreement? 8. Describe your companys return goods policy. 9. Describe your companys proposed implementation plan if you were to enter into an agreement with TRICARE Puget Sound and Alaska. 10. Does your company provide volume discounts? 11. Provide a history of back orders and/or recalls for this product group with dates, duration, cause, and how information was communicated to the customer, interim measures and final resolution. 12. Does your company distribute or manufacture your product line? If you distribute what brand(s) do you distribute? Product Criteria for Surgical face masks: 1. Does your company carry latex-free products? 2. Are your surgical masks environmentally safe? 3. What filtration and protection standards does your surgical face masks meet 4. Provide a list of the filtration levels associated with each type of mask you provide. 5. Do your surgical face masks provide protection (contain and filter) against droplets of microorganisms expelled from the mouth and nasopharynx during talking, sneezing and coughing? Describe how? 6. What is the average length of time your products can be worn and maintain the optimum level of protection? Please provide this information for each type of face mask you offer. 7. Are your products made of material that is liquid resistant and have low lint content? If so, what is the material? 8. Do you provide surgical face masks with fog-free solid shield (wrap around guard) for the entire face (eye, nose, mouth) 9. Do you provide products with low fog for eyeglass wearers? 10. Are your products comfortable to wear as well as breath in? Please describe. 11. Will your product withstand tearing under normal conditions? 12. Are the size and type of mask easily identifiable from the packaging? How? 13. Does your packaging allow for problem free dispensing? 14. Do your products with bands or stings allow for a secure tie that covers the mouth and nose in a manner that prevents venting?
