Description: The Chronic Disease Management Collaborative is a new VISN 23 collaborative that will serve as the framework for developing and implementing system-wide Chronic Disease Management (CDM) programs that address improved management of patients with COPD, CHF, and Diabetes. The Primary & Specialty Medicine Service Line has established a plan, which includes using the Improving Chronic Illness Care (ICIC) chronic disease management model for developing CDM programs and the IHI Breakthrough Collaborative as the method to achieve process change.
