Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. GovWorks is issuing this solicitation on behalf of the Department of Transportation/Federal Motor Carrier Safety Administration. The Request For Proposal (RFP) number is 60347. The resultant award is expected to be a labor hours type contract. The competitive RFP, specifications, RFP amendments, and all questions and answers related to this procurement will be made available via the Internet at http://www.govworks.gov/vendor/csolicit.asp under RFP number 1406-04-06-RP-60347. Potential offerors are responsible for accessing the web site. Ensure the RFP number and project title is referenced on all correspondence. The requirement is for the Federal Motor Carrier Safety Administration (FMCSA). FMCSA wishes to identify commercial motor vehicle driver factors that contribute to increased risk of large truck crashes. The study design should complement the FMCSA Large Truck Crash Causation Study addressing the relative crash risk associated with driver characteristics including personal data and traits, health and medical issues, driving performance and experience, and impacts of the work environment. The study should use a case-control sampling method in which the characteristics of crash-involved drivers (cases) are compared to those of non-crash-involved drivers (controls). The period of performance will be 18 months from the date of award. FOB shall be DESTINATION. The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. 52.212-2--Evaluation--Commercial Items. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price considered. 52.212-3, Offeror, Representations and Certifications-Commercial Items. The offeror must include a completed copy of this provision with their quote or refer the contracting officer to the electronic version of the Representations and Certifications posted to http://orca.bpn.gov/login.aspx. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items (including 52.203-6; 52.219-8; 52.219-14; 52.222-26; 52.222-35; 52.222-36; 52.222-37; 52.225-1; 52.225-3; 52.225-13; 52.219-6; 52.232-33; 52.222-21; and 52.222-43). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-18. The North American Industry Classification System (NAICS) code is 541990 with a small business size standard $6 million. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) and must include in the proposal over cover/transmittal letter, the CAGE Code that results from registering on the CCR website, the company's DUNS number and federal taxpayer ID number. One hard copy must also be delivered by mail to the address stated in the RFP located on the GovWorks website. Offerors are encouraged to discount labor rates in initial proposals. All requests for clarifications, questions as well as proposals must be e-mailed to Mr. Jay Zagursky at Jay.Zagursky2@mms.gov.
