- The Centers for Disease Control and Prevention intends to procure uniform rental services for specified activities within the Agency. The proposed contract resulting from this solicitation shall be Indefinite Delivery-Indefinite Quantity, Firm-Fixed Price with a base year followed by four (4) option years. The purpose of the proposed contract is for the Contractor to provide all labor, materials, supplies, transportation and equipment necessary to provide uniform rental services for employees of the Facilities Engineering Office (FEO), the Design and Construction Management Office (DCMO), and the Physical Security Activity (PSA). Uniforms will be required throughout the year. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The incorporated provisions and clauses are those in effect through the current Federal Acquisition Regulations and the Centers for Disease Control and Prevention Regulations. The Government intends to evaluate offers and award a contract without discussions with offerors providing the requested service. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. The North American Industry Classification System (NAICS) code is 812332 and the Size Standard is $13,000,000. Competition for this requirement is FULL AND OPEN to all interested business concerns. To successfully meet the performance requirements of this contract, the contractor must provide services from an established location within the Atlanta Metropolitan Area; and, within a 60-minute commute of the CDC Roybal Campus pickup/delivery locations specified in the Statement of Work. The Contractor must be able to show prior successful experience providing services similar in size, scope and complexity to our requirement. Any firm that believes it is capable of furnishing the Government with the service presented in this synopsis should submit four (4) copies of a proposal to the address below in response to this Request For Proposal (RFP). The proposal shall include itemized pricing, prompt payment terms, the F.O.B. Point (Destination or Origin as applicable), Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNs), and description of the service proposed. The following FAR clauses and provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors-Commercial Items (JAN 2005). Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications -Commercial Items (MAR 2005). The FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items (SEP 2005) is applicable. FAR provision 52.214-34, Submission of Offers in the English Language (APR 1991); FAR provision 52.214-35, Submission of Offers in U. S. Currency (APR 1991); FAR provision 52.217-5, Evaluation of Options (JUL 1990); FAR clause 52.217-8, Option to Extend the Services (NOV 1999); FAR clause 52.217-9, Option to Extend the Term of the Contract (MAR 2000); FAR clause 52.249-2, Termination for Convenience of the Government (Fixed-Price)(MAY 2004) and FAR clause 52.249-8, Default (Fixed-Price Supply and Service)(APR 1984). The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (JUL 2005) applies to this acquisition. The proposal shall include reference to RFP No. 2006-N-08297. Proposals will be due 30 days after the posting of this announcement or January 13, 2006, whichever is later, by 12:00 PM (local time). NOTE: The Offeror must have an active registration with the Central Contractor Registration (www.ccr.gov) at the closing of this RFP in accordance with FAR clause 52.204-7.
