- Supply Commodity Expendable Food Supplies for Blanchfield Army Community Hospital (BACH), Fort Campbell, KY. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as a Request for Quotes, W91248-06-R-A001. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-06. Acquisition is Set Aside for Small Business. The NAICS Code is 424420 and the small business size standard is 500 employees. Performance Work Statement (PWS) and Workload Data are provided as attachments. The Workload Data document also reflects the activitys estimated usage and quantities for the activitys requirement. Performance period will be a base period of twelve months, 1 January 2006 through 31 December 2006, with four (4) 12-months option periods. The provision at 52.212-1 Instructions to OfferorsCommercial Items, is applicable and is addended as follows: Paragraphs (b)(4), (b)(5), (d), (e), and (h) are deleted. This provision is further addended to incorporate 52.252-1 Solicitation Provisions Incorporated by Reference (Fill-in information - www.gsa.gov); 52.216-1 Type of Contract (Fill-in information, firm fixed-price requirements ); 252.204- 7001 Commercial and Government Entity (CAGE) Code Reporting; and 252.209- 7001 Disclosure of Ownership or Control by the Government of a Terrorist Country. The provision at 52.212-2 EvaluationCommercial Items is applicable; the following factors shall be used to evaluate offers: Technical (Contractors Product List and capability of this list to meet the needs of the Government), Past Performance, and Price. Technical (Contractors Product List) and Past Performance, when combined, are slightly more important than Price. The Government intends to make a single award to the responsive and responsible offeror whose offer is most advantageous to the Government considering price and non-price related factors. Offerors are instructed to submit completed copies of FAR Provision 52.212-3, Offeror Representations and CertificationsCommercial Items, and DFARS Provision 252.212-7000 Offeror Representations and CertificationsCommercial Items, or a statement that Offerors Representations and Certifications may be accessed at the ORCA website, with offers. Offerors should also submit their Product List with prices per case and past performance (brief description of services provided, dollar amount and length of contracts or agreements, points of contact and telephone numbers and/or e-mail addresses) for similar requirements. Offerors are instructed to submit the attached Past Performance Survey to customers where offeror is or has (within the past 3 years) performed the same or similar services and request completion.
