- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation for 100% small business set-aside utilizing the associated North American Industry Classification System code 624190, with a small b usiness size standard of $6 million. This requirement is for one (1) year from date of award with four (4) option years. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. Provisions at 52.212-1, Instructions to Offerers Commercial Items, apply to this acquisition. Offerors shall include with their offer a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items; 52.212-4, Contract Terms and Conditions Commercial Items. Provisions at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items; 52.203-6 (Alt I), Restrictions on Subcontractor Sales to the Government; 52.225-13, Rest rictions on Certain Foreign Purchases and 52.233-4, Applicable Law for Breach of Contract Claim are applicable to this acquisition. Provisions at 52.217-8, Option to Extend Services; and 52.217-9, Option to Extend the Term of the Contract also apply to th is acquisition. The Nashville District U.S. Army Corps of Engineers intends to issue a Request for Quote (RFQ) - W912P5-06-T-0005 for the acquisition of Employee Assistance Program (EAP) services to provide employees and their families with appropriate prevention, treatment, and rehabilitation programs and services for alcohol and/or drug abuse, family or marital pro blems, stress related situations, and legal or financial difficulties which may adversely impact on job behavior, performance, attendance and/or conduct. Details are included in the attached Statement of Work. Contractor shall submit prices for the all C ost Line Item Numbers (CLINs) (rates should be inclusive of all services requested) (See Attached). In addition, contractor shall submit a technical proposal. Facsimile and e-mail offers WILL be accepted. All offers must arrive at the place and by the t ime specified. No telephone (verbal) offers will be processed. All offers shall be clearly marked with the quotation number, offerors name and address, point of contact, phone number, fax number, and e-mail address (if available). Award will be made only to contractors who have registered with the Central Contractor Registration at www.ccr.gov and the Online Representations and Certifications Application (ORCA) at http://orca.bpn.gov.
