- Description
- Pre-Solicitation Notice. Construction of a new dental suite addition to an existing health care facility, the Chemawa Indian Health Center located in Salem, Oregon. Work consists of, but is not limited to, site preparation, utility connection, existing pavement and sidewalk alteration, selective demolition, and construction of the new addition, approximately 474 square meters (5,100 square feet.) The new space will be single-story, metal framed, with brick veneer exterior, a flat, single-ply membrane roof, and slab on grade foundation with utility corridor for medical air and gas piping. The performance period will be approximately 270 calendar days after the notice to proceed. The general dollar magnitude of the project is between $1,000,000 and $5,000,000. The proposed contract is 100% set-aside for small business concerns. All responsible sources may submit a bid that shall be considered by the agency. Bid documents will be available on or about October 18, 2005. The drawings will be issued in hard copy only. A non-refundable fee of $45.00 per set of bid documents is required. The fee must be provided in the form of a cashier’s check, certified check or postal money order, made payable to Department of Health and Human Services. Prime contractors and subcontractors should submit a request for plans and specifications before October 18, 2005 to Casey LaFran, Procurement Technician by regular mail (address above). The request should include contact information, and a statement as to whether the firm is a prime or subcontractor. No telephone requests for solicitations or posting status. NOTE: THIS NOTICE MAY HAVE POSTED ON FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (04-OCT-2005).
