Description
This amendment is being done to extend the response date from Dec 12 to Dec 16th 2005. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotes (RFQ). Submit written offers (oral offers will not be accepted) with a resume, on RFQ F2U0306168A100. This contract will be let for one basic period, and four option year periods. Please include pricing for four years out for this service if option is exercised at that time. Please contact SSgt Hanson if you have any questions. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-01 and Defense Federal Acquisition Regulation Supplement change notice 20050222. It is the offeror’s responsbility to be familiar with applicable clauses and provisions. This procurement is being issued as 100% small business set-aside. The North American Industry Classification System (NAICS) Code is 711130 and the size standard of 500 employees applies to this procurement. The requirement is to purchase Contemporary Protestant Leader Service to complete the following duties: LUKE AFB 1100 CONTEMPORARY PROTESTANT WORSHIP LEADER STATEMENT OF WORK 1. Duties And Responsibilities: The Contemporary Protestant Worship Leader will: a. Recruit, organize and prepare a praise team for the 1100 Contemporary Worship Service. b. Lead congregational singing as requested by the Service chaplain. c. Select and purchase musical needs for choir. d. Plan annual Choir Retreats to enhance team’s performances and spiritual community. e. Attend annual Parish Advisory Council planning retreat to establish yearly schedule of events. f. Schedule facility needs with NCOIC or appropriate Chaplain Assistant. g. Meet monthly with Service chaplain to review plans for worship and address praise team’s needs. h. Not exceed 175 annual units. Three units will be authorized each week -- 1 performance, 1 rehearsal, and 1 preparation. i. Arrive no later than 20 minutes prior to the start of the service or rehearsal. j. Assist in other musical programs performed within the Protestant parish. k. Be neatly attired, well groomed, and act in a professional manner. 2. Qualifications: The Senior Protestant Chaplain will conduct an interview and an audition. Preferred requirements are a Christian with Bachelor or Art degree; minimum of 2 years experience as a Praise Team Leader; and able to provide instruction in music reading to members of praise team. 3. Rate Of Pay: Rates will be based on a competitive, sealed bid for a per unit basis. A unit is defined as a scheduled performance or rehearsal held at the chapel facility lasting a minimum of one hour. The Senior Protestant chaplain in concert with the pastor of the service decides how many units are authorized for the specific activity. Time required to coordinate, organize and plan musical activities is considered in the establishment of the rate of pay. Contractor is paid for services actually performed and cannot exceed the total number of units authorized in paragraph one. 4. Equipment/Supplies: Requirements for music supplies are submitted in writing (Fund requests Forms) to the chaplain responsible for the service at least fourteen (14) days in advance of the required date. Contractor will prepare an annual budget for such supplies and equipment. 5. Security Of Facility: The contractor will ensure after each rehearsal the areas used are returned to the original condition and the facilities are properly secured. For each weekend worship service he/she needs only to ensure the choir/practice area used is neat and returned to its original condition. This requirement is for delivery FOB Destination to Chapel Building Number 799 Luke Air Force Base, Arizona 85309. The following clauses and provisions are incorporated by reference and are to remain in full force in any resultant purchase order. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, (Jan 2005) applies. The clause at FAR 52.212-4,Contract Terms and Conditions-Commercial Items and FAR 52.212-5 (Dev), Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation: 52.219-6 - Notice of Total Small Business Set-Aside; 52.222-3 - Convict Labor; 52.222-19 - Child Labor—Cooperation with Authorities and Remedies; 52.222-21 - Prohibition of Segregated Facilities; 52.222-26 - Equal Opportunity; 52.222-36 - Affirmative Action for Workers with Disabilities; 52.225-1 – Buy American Act—Supplies; 52.225-13 - Restrictions of Certain Foreign Purchases; 52.233-3 - Protest After Award; 52.242-15 - Stop-Work-Order; 52.247-34 - F.O.B. Destination; 52.253-1 - Computer Generated Forms; 52.252-2 - Clauses Incorporated by Reference; and 52.252-6 - Authorized Deviations in Clauses. The clause at DFARS 252.212-7001 (Dev), Contract Terms and Conditions Required to Implement Statues or Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation: DFARS 252.204-7003 - Control Of Government Personnel Work Product, 252.204-7004 - Required Central Contract Registration Alt A; 252.219-7011 - Notification to Delay Performance; 252.232-7003 - Electonic Submission of Payment Requests; 252.243-7001 - Pricing of Contract Modifications; 252.246-7000 - Material Inspection and Receiving Report; 252.247-7023 - Transportation of Supplies by Sea Alt III. 52.212-2 -- Evaluation -- Commercial Items. Evaluation -- Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: -Technically Acceptable (Interview, Audition, Resume) -Low Price (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.(End of Provision) To be eligible to receive an award resulting from this solicitation, contractor must (No Exceptions) be registered in the DoD Central Contractor Registration (CCR) database.
