Description
The Spokane, VA Medical Center (VA) located at; 4815 N Assembly Street, Spokane, Washington, 99205 is seeking off-site contractor(s) interested in providing all supplies, equipment, personnel and services necessary to perform all examinations to include; Magnetic Resonance Imaging (MRI), Mammography (Mammo) and after Close Of Business (COB) hours Computed Tomographic Scans (CT, Ultra Sound (US) and Nuclear Medicine (NucMed) imaging studies, within a 15 mile radius of the Spokane VAMC. All services shall meet or exceed VA/JCAHO standards. Verified reports for all exams shall be provided within 48 hours of the day of examination to fee- basis. For all after COB exams the contractor shall provide a written interim report. The interim report shall be returned with patient to the Spokane VAMC. Verified reports shall be sent with the second set of hardcopy images (except mammography images) to fee-basis at the Spokane VAMC within 48 hours of examination. A final, verified original report with 'signature on file' entry shall be sent to Diagnostic Imaging within 10 days of date of exam. Offers will be considered only from offerors who are regularly established in the business called for and who are financially responsible and have the necessary equipment and personnel to furnish service in the volume required in accordance with all terms and conditions under this contract. Successful offerors shall meet all requirements of Federal, State and City codes regarding operations of this type of service. The contract will be for a one year period with four separate one year option periods for a total contract period of 5 years. The cost range for this contract is estimated between $2 Million and $5 Million. The NAICS code for this procurement is 621512, ?Diagnostic Imaging Center? with a size standard of $11.5 million. A company?s average annual receipts over the last 3 years cannot exceed $11.5 million to be considered a small business. The VA anticipates award of one contract for all items required. Interested contractors shall provide the following information; 1) Company Name, 2) Point of contact and contact information to include but not limited to phone and e-mail address, 3) Size of company, based on NAICS code and size standard listed above, small businesses should also identify if qualified as; a) Service Disable Veteran Owned, b) Veteran Owned, c) Certified 8(a) Disadvantaged Owned, d) Certified Small Disadvantaged Owned, e) Certified HUB Zone Owned , and/or f) Woman Owned, and 4) Statement of how the contractor would propose to service the contract,. All responses shall be submitted no later than 3:00 PM PDT, December 30, 2005 to the Contracting Officer, Rich Hague at richard.hague@med.va.gov.
