- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. Solicitation number W91248-06-Q-0011 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquis ition Circular 2005-06. Acquisition is set aside for small business concerns. The NAICS Code is 711130 and the small business size standard is $6.0 Million. Performance period is for one 12-months base period and two (2) 12-months option periods. Antic ipated start date is on or about 1 January 2006 through 31 December 2006 (base period). The FAR provision at 52.212-1 Instructions to Offerors-Commercial Items is applicable and is addended as follows: Paragraphs (b)(4),(e)(5), (d), and (e) are deleted. The Government may make multiple awards to the responsive and responsible offerors whose offers are most advantageous to the Government considering price and non-price related factors. The FAR provision at 52.212-2 Evaluation-Commercial Items is applica ble (e valuation factors are Technical (Experience) and Price. Technical factor is significantly more important than Price. A minimum of two years experience as a church musician (pianist/keyboard) is required. The successful offeror must be registered in Central Contractor Registration (CCR). FAR 52.232-19, Availability of Funds for the Next Fiscal Year (Apr 1984), Funds are not presently available for performance under this contract beyond 31December 2006. The Governments obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for perf ormance under this contract beyond 31 December 2006, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer. 18. Place of Performance will be at the following three (3) Chapels: Soldiers, Memorial, and Grace Chapels, Fort Campbell, KY 42223. Protestant and Catholic Services will be provided at Soldiers Chapel and Protestant Services at Grace and Memorial Chapel s. Offers are due no later than 29 December 2005, 12:00 P.M. (CST). Acquisition is 100% set-aside for small business. SCHEDULE OF INSURANCE: a. The Contractor shall, at its own expense, provide and maintain during the entire performance period of this contract at least the kinds and minimum amounts of insurance required below: Workmans Compensation and Employers Liability Insurance $100,000.00 General Liability Insurance for Bodily Injury Liability: Minimum Per Occurrence $500,000.00 Automobile Liability Insurance: Minimum Per Person $200,000.00 Minimum Per Occurrence for Bodily Injury $500,000.00 Minimum Per Occurrence for Property Damage $ 20,000.00 b. Before commencing work under this contract, the Contractor shall certify to the Contracting Officer in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Governments interest shall not be effective: (1) For such period as the laws of the state in which this contract is to be performed or prescribed, and (2) Until 30 days after the insurer or the contractor gives written notice to the Contracting Officer, whichever period is longer. c. The Contractor shall insert the substance of the clause entitled Insurance-Work on a Government Installation (FAR 52.228-5) in subcontracts under this contract that requires work on a Government installation and shall require subcontractors to provide and maintain the insurance required above.
