Description
The Veterans Affairs Medical Center, 4100 West Third Street, Dayton OH 45428, intends to negotiate on a sole source basis with Clinical Technology Inc., One Corporation Center, Broadview Heights OH 44147, for purchase of the equipment as described herein. The NAICS code is 339113. The following descriptions are Brand name or Equal. Item 1, Radiofrequency Interstitial Tissue Ablation System (RITA) consisting of the following: (1) RF generator 1500X (version 8.21); (1) Power cord; (1) foot switch; (2) main cable (autoclavable); (1) user/service manual, Clinical Technology Model 700-101731. Includes RITA Intelliflow pump (700-102684). Item 2, Starburst XL (700-101930), 9 array 3-5 cm, 10 cm (includes two dispersive pads per needle). Item 3, Starburst SemiFlex (700-102615), 25 cm (includes two dispersive pads per needle). Item 4, Radiofrequency Lesion Generator NeuroThermo Model RFG-NT-25 w/Modes: Impedence Check and simulator conventional Thermal RF Lesion & Pulsed RF IDET and Disctrode Protocol. Item 5, IDET Access cable (IAC)-1. Item 6, Disposable grounding pads (RF-DGP). The Government believes that only responsible source can provide the supplies and intends to solicit and negotiate with only one source under the authority of FAR 6-302-1(b)(4). This notice of intent is not a request for competitive proposals. No solicitation document is available and telephone requests will not be honored. However, if any firm believes that it can meet the requirement, supporting evidence must be furnished in sufficient detail to demonstrate the ability to provide this equipment. Any supporting evidence and/or correspondence must be addressed to: VA Medical Center, Contracting Officer (90C), 4100 West Third Street, Dayton OH 45428, or via fax to 937-262-5974,by 12:00 NOON on January 6, 2006. CONTRACTORS ARE ADVISED THAT ELECTRONIC RESPONSES TO THIS NOTICE MAY NOT BE RESPONDED TO. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the VA will proceed with the sole source negotiation with Clinical Technology Inc. No set aside applies to this procurement.
