<DESC> This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13.5, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both FEDBIZOPPS and NECO (http://www.neco.navy.mil/ ). The RFQ number is N00140-06-Q-0023. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-05 and DFARS Change Notice 20051114. It is the contractor’s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: www.acqnet.gov/far and http://www.acq.osd.mil/dpap/dfars/index.htm . The NAICS code is 622110. This is a competitive, unrestricted action in support of the National Naval Medical Center, Bethesda, MD for the supply of 550 does of F-18 FDG (Fluorodeoxglucose) 10-15 mCi/ dose for use in Positrom Emission Tomography imaging. The performance period for this requirement will consist of a base period of 9 months starting 1 Jan 06 through 30 Sept 06, and will also have up to four one year option periods consisting of 12 months starting 1 Oct through 30 Sept. The estimated option quantity is approximately 750 doses per option year. Doses of FDG-18 will have a shelf life of approximately 5 hours after manufacture. It is anticipated that approximately 12 to 15 doses of FDG18 will be required weekly. The FDG 18 must be delivered to the National Naval Medical Center, Nuclear Medicine (Radiation Therapy Division) no later than 30 minutes prior to a procedure. It is noted that at times, multi-deliveries to the National Naval Medical Center may be required. Delivery of this material must be IAW all Federal, State and Local regulations. Contractor shall adhere to FAR clause 52.223-3 Hazardous Material Identification and Material Safety Data (Jan 97) and FAR clause 52.223-7 Notice of Radioactive materials also date (Jan 97). The National Naval Medical Center, Bethesda MD is hereby authorized ordering authority under this requirement. The National Naval Medical Center Bethesda shall verbally notify the successful contractor of the expected requirements for the following day. It is noted that all though not expected at times an urgent same day delivery may be required. Verbal orders will be confirmed in writing on a monthly basis on a Delivery order that is prepared by the Supply Department National Naval Medical Center, Bethesda MD. The delivery order will be used to match vendor invoices and to generate vendor payment. The following FAR provision and clauses are applicable to this procurement: FAR 52.216-18 entitled Ordering (Oct 95) (a) issued from 1 Jan 06 thru 30 Sept 06, 52.216-21 entitled Requirements (Oct 95) (f) 30 Sept 06, 52.212-1, Instructions to Offerors - Commercial Items; 52.212-3 ALT I, Offeror Representations and Certifications - Commercial Items; 52.212-4, Contract Terms and Conditions - Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (7), (8), (9), (13), (14), (15), (16), (17), (18),(19),(29). Quoters are reminded to include a completed copy of 52.212-3 ALT I with quotes. All clauses shall be incorporated by reference in the order. Additional contract terms and conditions applicable to this procurement are: 252.212-7000, Offeror Representations and Certifications - Commercial Items and 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items including 52.203-3 entitled Gratuities, and 252.243-7002 entitled Request for Equitable Adjustment. FAR 52.217-9 (Mar 2000) The Government may extend the term of this contract provided notice of the Government’s intention to exercise said option is notified to the successful contractor prior to the expiration date of the current contracts period of performance. All responsible sources may submit a quote which shall be considered by the agency. 52.212-2, Evaluation - Commercial Items is applicable to this procurement. While price will be a significant factor in the evaluation of offers, the final contract award will be based on a combination of factors - a combination of price and past performance, with past performance being considered more important than price. Central Contractor Registration (CCR). Quoters must be registered in the CCR database to be considered for award. Registration is free and can be completed on-line at http://www.ccr.gov/ . All quotes shall include price(s), FOB point, a point of contact, name and phone number, GSA contract number if applicable, business size, and payment terms. Quotes over 10 pages in total will not be accepted by facsimile. Each response must clearly indicate the capability of the quoter to meet all specifications and requirements. The quoter shall submit, as part of its quote, information on previously performed contracts or on-going contracts that are similar in scope complexity and magnitude to the statement of work/item description in the solicitation performed for Federal, State or local Governments, and for commercial firms. Information shall be provided on your most relevant and will not exceed three sources. The information submitted shall include a contract number, the total dollar value, and period of performance of the contract. A POC knowledgeable about your performance under that contract. A description of the services performed under that contract, the # of and type of quality, delivery issues associated with that contract and how they were resolved, and any other information the quoter considers relevant to past performance of the referenced contract. The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources including sources outside of the Government. Quoters lacking relevant past performance history will not be evaluated favorably or unfavorably on past performance. However, the quote of a company with no relevant past performance history, while not rated favorably or unfavorably for past performance, may not represent the most advantageous quote to the Government and thus, may be an unsuccessful quote when compared to the responses of other quoters. The quoter should provide the information requested above for past performance evaluation, or affirmatively state that it possesses no relevant directly related or similar past performance. The assessment of the quoter’s past performance will be used as a means of evaluating the relative capability of the responder and other competitors to successfully meet the requirements of the RFQ. The Government will give greater consideration to the contracts that the Government feels are most relevant to the RFQ. The closing date of this requirement is 1 Dec 05 at 4:00 PM.
