- THE COMBINED SYNOPSIS/SOLICITATION IS HEREBY AMENDED (2nd Amendment). The complete text is replaced by the following: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals a re being requested and a written solicitation will not be issued. W912CM-06-T-0039 is a request for quotation (RFQ), using the simplified acquisition procedures under FAR Part 13.5 (Test Program for Certain Commercial Items); its document and incorporated provisions and clauses are those in effect through the Federal Ac quisition Circular 2005-06. For procurement is ITEM 0001: One year Foundry TechNet Silver Support (which provides for telephonic assistance 24 hours a day, 7 days a week, documentation updates, software upgrades, and a 3-day hardware replacement) for maintenance of 5th Signal Comm and European Theater owned Foundry Switches, on a firm fixed-price basis. For the Performance Work Statement and the list of equipment (Attachment 1), contact the point of contact named below. PERFORMANCE PERIOD. One year starting 07 Jan 2006. Award is subject to Availability of Fiscal Year 06 funds. Contract will be awarded only when funds become available. The following provisions in their latest editions apply to this solicitation: (1) FAR 52.212-1, Instructions to Offerors with the addendum that: The offeror shall submit as their offer: (a) Proposed price, (b) completed copy of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items; and (c) Cage Code, DUNS and TI N. Offeror may email or fax their offer. (2) FAR 52.212-2, Evaluation, with the following basis of award: Award will be made to the low responsive offeror with technically acceptable offer. Past performance will be evaluated as part of the responsibility determination. For technical acceptabili ty, the offeror must be able to provide the required Foundry maintenance services, as authorized or licensed by Foundry. The companys capability will be confirmed by the company Foundry. Assuming the offeror is responsible and the offer is technically ac ceptable, the lowest price is the most important factor. (3) FAR 52.212-3, Offerors Representations and Certifications; a completed copy of which shall be submitted with the offeror (4) FAR 52.212-4, Contract Terms and Conditions; (5) FAR 52.212-5 (Deviation), Contract Terms and Conditions Required to Implement Statutes or Executive Orders.
