Description
THIS IS A COMBINED SYNOPSIS, (REQUEST FOR QUOTATION WILL NOT BE MAIL OUT), TO PROVIDE PROFESSIONAL CONSULTING FUNCTIONS AND SERVICES TO THE OFFICE CHIEF EXECUTIVE OFFICER AND TO THE FORT DEFIANCE INDIAN HOSPITAL BOARD, INC. (FDIHB) OF REASEARCH RESULTS TO THE SERVICE UNIT ADMINISTRATION. THESE SERVICES WILL REQUIRE THE CONSULTANT TO WORK DAYS, EVENINGS, WEEKENDS AND HOLIDAYS. THE CONTRACTOR SHALL PERFORM ROUTINE ADMINISTRATIVE FUNCTIONS AND SERVICES AS THE HEALTH BOARD CONSULTANT ON A DAILY BASIS AS ASSIGNED BY THE CHIEF EXECUTIVE OFFICER OR HIS DESIGNEE. THE CONTRACTOR SHALL HAVE CONTACT WITH HEALTH BOARDS, COMMUNITY PEOPLE, STAFF, TRIBAL ENTITIES, SERVICE UNITS, AREA STAFF, AND PATIENTS OF THE FORT DEFIANCE INDIAN HOSPITAL, FORT DEFIANCE, AZ. THE CONTRACTOR SHALL PROVIDE NECESSARY TRAINING ON P.L. 93-638 TO THE FDIHB, FDSU STAFF AND COMMUNITIES WITHIN THE FORT DEFIANCE SERVICE UNIT. THIS WILL PREPARE THE FDIHB AND STAFF TO BETTER UNDERSTAND P.L. 93-638 PROCESSES AND FUNCTIONS. ALL WORK PERFORMED SHALL BE COMPLETED THROUGH FISCAL YEAR 2006. THE WORK SHALL BE COMPLETED IN PHASES ON A QUARTERLY BASIS. FIRST PHASE - DATE OF AWARD THROUGH MARCH 31, 2006. DELIVERABLES SHALL BE SUBMITTED ON OR BEFORE MARCH 31, 2006. CONTRACTOR SHALL ADHERE TO THE OUTLINED TASK LIST AND SUBMIT THE FOLLOWING DELIVERABLES: 1. FOR THE FIRST FOUR WEEKS, A REVIEW OF ALL THE HEALTH SERVICES WITHIN THE FORT DEFIANCE INDIAN HOSPITAL AND NAHATA DZIIL HEALTH CENTER WOULLD BE COMPLETED IN ORDER TO GET A PERSPECTIVE OF LAYING OUT A PHASE IN MODEL OF P.L. 93-638. (a) WEEKLY PROGRESS REPORT OF THE NUMBER OF MEETINGS AND CONTACTS MADE IN PROGRESSING WITH THE REVIEW OF SERVICES. (b) THE REPORT SUBMITTED TO THE OFFICE OF THE CEO. 2. A THOROUGH RESEARCH OF THE IMPACT OF A MODEL CONTRACT VERSUS A TOTAL CONTRACT WOULD BE EVALUATED. THIS TASK MAY REQUIRE A SUB-CONTRACTOR TO ASSIST WITH THIS TASK. (a) WEEKLY PROGRESS REPORT OF THE ASSESSMENT, DEVELOPMENT, DESIGN, IMPLEMENTATION AND EVALUATION OF A P.L. 93-638 CONTRACT. (b) REPORT SHALL HAVE THE NUMBER OF CONTACTS. (c) REPORT SHALL HAVE THE NUMBER OF MEETINGS. (d) REPORT SHALL HAVE THE NUMBER OF STAFF AND HEALTH BOARD MEMBERS. 3. DEVELOP A CONTINUOUS TRAINING PLAN WITH TIMELINE AND SCHEDULE FOR THE SIXTEEN (16) CHAPTER HOUSES, DISTRICT 17 AND DISTRICT 18, AND FORT DEFIANCE AGENCY COUNCIL. (a) WEEKLY PROGRESS REPORT OF THE TRAINING TOPICS, DATES OF THE PRESENTATIONS AND LOCATIONS. (b) REPORT SHALL HAVE THE NUMBER OF CONTACTS. (c) REPORT SHALL HAVE THE NUMBER OF MEETINGS/TRAINING. (d) REPORT SHALL HAVE THE NUMBER OF PARTICIPANTS - CHAPTER HOUSE, DISTRICTS AND AGENCY MEMBERS. SECOND PHASE - APRIL 1, 2006 THROUGH JUNE 30, 2006. DELIVERABLES SHALL BE SUBMITTED ON OR BEFORE JUNE 30, 2006. 4. DEVELOP A TRAINING PLAN WITH TIMELINE AND TRAINING SCHEDULES FOR THE TECHNICAL, MANAGERIAL AND PROFESSIONAL STAFF AT FORT DEFIANCE INDIAN HOSPITAL AND THE FORT DEFIANCE INDIAN HOSPITAL BOARD, INC. (a) WEEKLY PROGRESS REPORT OF THE TRAINING WITH PROFESSIONAL, TECHNICAL AND MANAGERIAL STAFF OF FORT DEFIANCE SERVICE UNIT. THIS INCLUDES TRIBAL AND FEDERAL PARTINERS. (b) REPORT SHALL HAVE THE NUMBER OF TRAINING TOPICS. (c) REPORT SHALL HAVE THE NUMBER OF PARTICIPANTS PER TRAINING TOPICS. (c) REPORT SHALL HAVE THE NUMBER OF PARTICIPANTS PER TRAINING TOPICS. (d) REPORT SHALL HAVE THE NUMBER OF PARTICIPANTS PER HOSPITAL AND PER PARTNERS. 5. THE TRAINING PLAN SHALL COVER P.L. 93-638 UNDER THE INDIAN HEALTH SERVICE PROVISIONS; MODEL CONTRACTING VERSUS TOTAL CONTRACT; AND SELF GOVERNANCE COMPACT. (a) WEEKLY PROGRESS REPORT OF THE TRAINING REGARDING THE DIFFERENCES OF CONTRACTING UNDER P.L. 93-638. (b) REPORT SHALL HAVE THE NUMBER OF RESPECTIVE TOPICS OF P.L. 93-638. (c) REPORT SHALL HAVE THE NUMBER OF PARTICIPANTS - PER HOSPITAL, CHAPTERS, BOARD MEMBERS AND PARTNERS. 6. THE PARTICIPANTS (i.e. FDSU STAFF AND BOARD MEMBERS) SHALL RECEIVE AN ORIENTATION SESSION TOWARDS A READINESS STRATEGY TO CONTRACT UNDER P.L. 93-638. (a) WEEKLY PROGRESS REPORT OF THE STRATEGY PLAN FOR P.L. 93-638. (b) REPORT SHALL HAVE A WORKABLE DOCUMENT THAT IS RESENTED TO THE STAFF AND FDIH BOARD MEMBERS AS A MEANS OF AWARENESS AND TRAINING TOPICS. 7. A PROPOSAL FOR A MODEL CONTRACT FOR SERVICES SHALL BE DESIGNED, DEVELOPED AND IMPLEMENTED FOR REVIEW BY THE HEALTH AND SOCIAL SERVICES COMMITTEE OF THE NAVAJO TRIBAL COUNCIL AND BY THE INTERGOVERNMENTAL RELATIONS COMMITTEE OF THE NAVAJO TRIBAL COUNCIL. (a) WEEKLY PROGRESS REPORT SHALL HAVE A RESOLUTION IN PROGRESS FROM THE FORT DEFIANCE INDIAN HOSPITAL BOARD, INC. TO THE HSSC AND TO THE IGR COMMITTEE. (b) REPORT SHALL HAVE THE NUMBER OF CONTACTS AT THE HOSPITAL. (c) REPORT SHALL HAVE THE NUMBER OF CONTACTS WITH THE TRIBAL COMMITTEES. (d) REPORT SHALL HAVE THE NUMBER OF MEETINGS WITH THE LOCAL CHAPTERS, IF NECESSARY. THIRD PHASE - JULY 1, 2006 THROUGH SEPTEMBER 30, 2006. DELIVERABLES SHALL BE SUBMITTED ON OR BEFORE SEPTEMBER 30, 2006. 8. ESTABLISH AND IMPLEMENTATION PLAN WITH FORT DEFIANCE INDIAN HOSPITAL STAFF. ESTABLISH A CORE MANAGEMENT GROUP OF STAFF TO PLAN AND INITIATE THE IMPLEMENTATION PLAN. EACH DEPARTMENT STAFF SHALL RECEIVE AN ORIENTATION OF THE IMPLEMENTATION PLAN. (a) WEEKLY PROGRESS REPORT SHALL HAVE THE NUMBER OF TASK FORCE MEETINGS AS THE MEANS OF RECOMMENDING A DIRECTION TO THE FORT DEFIANCE SERVICE UNIT EXECUTIVE COMMITTEE AND TO THE FORT DEFIANCE INDIAN HOSPITAL BOARD, INC. (b) REPORT SHALL HAVE THE NUMBER OF ORIENTATION SESSIONS WITH THE MED-NURSE EXECUTIVE COMMITTEE. (c) REPORT SHALL HAVE THE NUMBER OF ORIENTATION SESSIONS WITH THE FORT DEFIANCE INDIAN HOSPITAL BOARD, INC. 9. DEVELOP A SITE VISITATION WITH OTHER TRIBAL CONTRACTED HOSPITALS - i.e. GILA RIVER, COEUR D'ALENE TRIBE'S BENEWAH MEDICAL CENTER, ETC. (a) WEEKLY PROGRESS REPORT OF THE POTENTIAL p.l. 93-638 SITES WITHIN THE SOUTHWEST AND WESTERN REGIONS OF THE UNITED STATES. (b) REPORT SHALL HAVE THE TRIP REPORT AND SIGNIFICANT INFORMATION RECEIVED AT THE TRIBAL ORGANIZATION OR TRIBAL GOVERNMENT SITE. (c) REPORT SHALL HAVE THE TRIP REPORT AND INDICATIONS OF DOCUMENTS RECEIVED FROM THE SITE THAT WOULD BE OF CONSIDERATION IN IMPLEMENTING P.L. 93-638. (d) REPORT SHALL HAVE A REVIEW OF THE BOARD POLICIES AND RECOMMENDATIONS FOR AMENDMENTS. 10. ESTABLISH A STRATEGY OF CONTRACT SUPPORT COSTS, WHICH MAY REQUIRE AN INDIRECT COMMUNICATION WITH CONGRESSIONAL AIDS AND THE WASHINGTON OFFICE OF THE NAVAJO NATION. (a) WEEKLY PROGRESS REPORT SHALL ASSESS THE OVERALL OPERATIONAL COST FOR FORT DEFIANCE SERVICE UNIT IN A BUDGET FORMAT. (b) REPORT SHALL HAVE A SPENDING PLAN PER DIVISION OF THE SERVICE UNIT. (c) REPORT SHALL HAVE A SEPARATION OF DIRECT CARE SERVICE COSTS AND INDIRECT SERVICE COSTS. CONTRACTOR SHALL BE PERSONALLY RESPONSIBLE FOR OWN HOUSING (LODGING), TRANSPORTATION (TRAVEL), PERSONAL BENEFITS, INSURANCE AND REPORTING WAGE OF EARNINGS TO THE IRS. CONTRACTOR SHALL BE RESPONSIBLE FOR ALL FEDERAL, STATE AND LOCAL TAXES SUCH AS THE NAVAJO NATION BUSINESS ACTIVITY TAX. THIS IS TOTAL SET-ASIDE FOR BUY INDIAN. 320 HOURS $_____TOTAL BID $_____.
