Description
The purpose of this amedment is to change the response date from: 20 Dec 2005 To: 21 Dec 2005 at 1600 hrs. Pharmacist to provide pharmacy services to the Government. Basic (12 months) and 4 option years. Period Of Performance: 01/02/2006-01/01/2007 Duties as stated in the Performance Work Statement. This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being required and a written solicitation will not be issued. The Performance Work Statement (SOW) and other requirements will be attached/posted. The following clauses are applicable to subject solicitation: FAR 52.212-1 (Instructions to Offerors), 52.212-2 (Evaluation - Commercial Items), 52.212-3 (Offeror Representations and Certifications - Commercial Items), 52.212-4 (Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items). PERFORMANCE WORK STATEMENT FOR 78th MEDICAL GROUP – STAFF PHARMACIST SECTION 1 DESCRIPTION OF SERVICES SECTION 2 SERVICE DELIVERY SUMMARY SECTION 3 GOVERNMENT FURNISHED PROPERTY AND EQUIPMENT SECTION 4 GENERAL INFORMATION SECTION 5 APPENDICES 1. DESCRIPTION OF SERVICE 1.1. SCOPE OF WORK – This statement of work describes the work to be performed in the pharmacy at the 78th Medical Group. The contractor shall provide a pharmacist to provide pharmacy services to Government beneficiaries eligible for healthcare. The contractor shall provide care at Robins AFB Pharmacy, also referred to as “military treatment facility” or “MTF” herein. Contractor shall furnish all labor, management, teaching, consultations, and reports, except as provided by the government as listed below. Contractor care shall cover the range of services provided in a civilian medical treatment facility. Performance shall be according to the requirements contained in this Performance Work Statement (PWS) and professional standards of the Joint Commission on Accreditation of Healthcare Organizations. 1.2. REQUIREMENTS - Contractor’s proposed staff members shall perform the following tasks: 1.2.1. ASSISTS IN PROCUREMENT, STORAGE, MANUFACTURE, DISTRIBUTION, CONTROL, AND EVALUATION OF DRUGS – The pharmacist shall ensure quality drugs are available for patients in the proper quantity at the correct time. This shall include but not be limited to: · Formulates specifications for the quality and source of all drugs procured for the pharmacy. · Supervises filling, labeling and dispensing of all drug orders. · Inspects stock supplies of drugs on all services at the Pharmacy. · Maintains an approved stock of antidotes and other emergency drugs. · Oversees the manufacture of bulk-compounded preparations if their procug tasks: 1.2.1. ASSISTS IN PROCUREMENT, STORAGE, MANUFACTURE, DISTRIBUTION, CONTROL, AND EVALUATION OF DRUGS – The pharmacist shall ensure quality drugs are available for patients in the proper quantity at the correct time. This shall include but not be limited to: · Formulates specifications for the quality and source of all drugs procured for the pharmacy. · Supervises filling, labeling and dispensing of all drug orders. · Inspects stock supplies of drugs on all services at the Pharmacy. · Maintains an approved stock of antidotes and other emergency drugs. · Oversees the manufacture of bulk-compounded preparations if their procurement from commercial sources is not possible or advantageous to the Air Force. · Oversees the unit dose drug distribution system. · Maintains patient drug profiles. 1.2.2. PROVIDES CONSULTATIVE SERVICES – The pharmacist shall provide consultative services. This shall include but not be limited to: · Establishes a central repository of drug information. · Communicates essential drug information to patients and health care personnel. · Acts as consultant regarding drugs for the health care staff. · Instructs health care personnel in pharmacy matters. 1.2.3. DOCUMENTATION – The Pharmacist shall prepare all documentation to meet or exceed standards in MDGI 44-8 to include but not limited to: timeliness, legibility, accuracy, content, and signature. 1.2.4. CONDUCTS TRAINING – The pharmacist shall serve in an advisory or teaching capacity. The pharmacist shall provide training to MTF staff in pharmacy related topics. 1.2.5. OTHER DUTIES – Pharmacists shall be primarily required to perform pharmacy services. When the services are not required, or the pharmacist’s services are more urgently required elsewhere, the pharmacist may provide support in other sections/departments of the MTF commensurate with their experience. 1.2.6. SPECIFIC DUTIES – The pharmacist shall administer pharmacy services for patients as scheduled by the Government. This shall include but not be limited to: · Maintains a collegial work environment conducive to good pharmaceutical care. · Implements Pharmacy and Therapeutics Function decisions. · Ensures maintenance of perpetual inventories on all controlled substances. · Recommends and implements a drug distribution system yielding maximum pharmacy control over requisitioning, storing, and dispensing drugs. 2. SERVICE DELIVERY SUMMARY Performance Objective PWS Para Performance Standard Prescription processing - Process prescriptions at a rate sufficient to maintain a reasonable patient waiting time while maintaining accuracy within Air Force standards 1.2.1 Accuracy standards to be maintained below 0.02% error rate Reports - Reports and correspondence are consistently presented effectively and address all pertinent issues and/or concerns.- Prescription Drug Interaction Warning Report and the Pharmacy Edits reports 1.2.3 Reports must be done each day for the previous work day by Close of business – 1630hrs Drug Inspection - Periodically inspects drugs for adequacy, overstocking, arrangement, storage, proper rotation, and evidence of deterioration. 1.2.1 Inspections must be completed monthly within the month tasked 3. GOVERNMENT FURNISHED PROPERTY AND SERVICE 3.1. GENERAL – The Government will provide, without cost, the facilities, materials, equipment, and services needed to work in the pharmacy. Government-furnished property shall be for use only in the performance of this agreement. The Government will provide routine scheduled preventive maintenance of government-furnished equipment in accordance with MTF instructions. 3.2. ADMINISTRATIVE SUPPORT. Contract personnel shall be authorized Composite Healthcare Computer System (CHCS) and computer access only to complete assigned tasks. Contractor’s use of CHCS, and a computer system is limited to official government business. 3.3. GOVERNMENT ORIENTATION/TRAINING. The Government shall provide training on Government provided forms and equipment, initial and continuing orientation. Training will be provided on the Composite Healthcare System, PharmAssist automation equipment, ScriptPro prescription dispensing robot, QMATIC, and AudioCare telephone refill system. 3.4. EQUIPMENT – Contract personnel shall have joint use of all equipment necessary to perform services required by this contract. 3.5. FORMS – The MTF will provide required Government forms used in performance of services. 3.6. SUPPLIES – All office/medical supplies currently and normally used in the MTF in providing the required services will be available for the healthcare worker’s use. The Government will furnish healthcare workers with appropriate Personal Protective Equipment (PPE). The Government will be responsible for any repair, cleaning, and inventory requirements for the PPE. The MTF does not provide other uniforms, laboratory coats, or other personal clothing. 4. GENERAL INFORMATION 4.1. SPECIAL REQUIREMENTS: · Contractor will demonstrate verbal skills to ensure effective interpersonal interactions. · All contractor staff will be well groomed and in appropriate clothing when in-patient care area and public areas of the MTF. · At no time will staff members provide clinical advice to patients. · Contractor shall maintain open and professional communication with members of the MTF. 4.2. ABSENCES AND LEAVE 4.2.1. ARRANGE FOR REPLACEMENT STAFF – The contractor shall advise the CO, supervisor, or designee about absences due to illness or incapacitation. If the contractor is absent for three or more consecutive days due to illness, the contractor may be required to provide written documentation from a qualified health care provider that he or she is free from communicable disease and the cause of the worker’s current illness. The Government reserves the right to examine and/or re-examine any worker who meets this criterion. 4.2.2. The contractor shall notify the MTF 30 days in advance of scheduled leave and accommodate the MTF schedule for leave periods of a week or more. 4.3. CONTINUITY OF SERVICES. If routine services are disrupted for more than three (3) consecutive scheduled shifts, the Government reserves the right to procure such services from another source until the Contractor restores routine services. When the Government exercises its right to procure these services from another source, the Government will reduce the Contractor’s invoice at an equivalent amount to that incurred. A copy of the other source’s service ticket will be used as the basis for this reduction. The Government will furnish the Contractor a copy of this ticket upon the Contractor’s request. 4.3.1. Contractor staff shall be deemed to be non-critical, non-essential personnel and will comply accordingly with all closings for severe weather or other emergencies, which will be at the sole discretion of the government. Contractor shall maintain the required service schedule, unless notified by the Task Manager that a closing due to weather conditions or other emergency is authorized. During crisis or emergencies, this effort is not applicable to the DODI 3020.37, Continuation of Essential DoD Contractor Services During Crisis. 4.4. FORMAL EDUCATION - The following minimum requirements are required to be considered for a position under this agreement. The individual must possess a Bachelor of Science or Doctor of Pharmacy degree from an accredited College of Pharmacy. Copies of the documents listed below shall be submitted 30 days from receipt of notice of contract award. The following is a list of requirements: 4.4.1. LICENSURE/REGISTRATION – The Contractor shall possess a valid unrestricted license to practice pharmacy from the state where the MTF is located. Contractor must provide proof of current licencensure within state guidelines 4.4.2. Copy of current Basic Life Support (BLS) mandatory. 4.4.3. EXPERIENCE – The Contractor shall have provided pharmacy services a minimum of eleven months within the past fifteen months. 4.4.4. CONTINUING MEDICAL EDUCATION (CME) REQUIREMENTS – Health care providers registered or certified by national/medical associations shall continue to meet the minimum standards for CME to remain current as prescribed in AFI 41-117. CME shall be obtained at no additional cost to the Government and shall be reported to the QAP annually on the first normal duty day in January for the previous calendar year. Periodic CME may be conducted at the MTF and will be available, at no cost, to any health care provider desiring to attend. 4.5. HEALTH REQUIREMENTS: 4.5.1. Certification of current physical examination for each employee. The certification shall contain a signed statement by the examining physician stating that the employee is free of any contagious diseases. Physical examination must be current as of 12 months prior to application for privileges. 4.5.2. Contract personnel providing services under this contract shall receive a preemployment physical examination prior to commencement of work and annually thereafter. Contract personnel report to the Contractor’s physician to receive a pre-employment examination and immunizations/shots prescribed by the MTF. 4.5.3. Not later than five (5) working days prior to commencement of work, certification shall be provided to the Contracting Officer’s representative that contract personnel have completed medical evaluation required above. This certification shall state the date on which the examination was completed, the doctor’s name that performed the examination, and a statement concerning the physical health of the individual. The certification shall also contain the following statement: “(name of contract employee) is free from contagious diseases to include but not limited to Tuberculosis, Hepatitis, and Venereal Disease.” 4.5.4. Also, as a condition of employment, OSHA requires that all contract personnel who will have occupational exposure to blood or body fluids, or other potentially infectious materials, shall receive Hepatitis B vaccine, sign a voluntary declination, or have documented proof of immunity to Hepatitis B infection. Personnel who sign declinations may change their minds at anytime and receive the Hepatitis B vaccine without penalty. 4.5.5. It is the Contractor’s responsibility to report (to the appropriate MTF staff member) all information necessary to assure MTF records can be maintained correctly, and therefore comply with the JCAHO, OSHA, and CDC health records requirements. 4.5.6. EMERGENCY HEALTHCARE – The MTF will provide emergency care for contract personnel for injuries occurring while on duty in the MTF. These services will be billed to the Contractor at the current full reimbursement rate. 4.6. HOURS OF OPERATION. The pharmacist will be on duty located in the pharmacy in the 78th Medical Group for an average of approximately 40 hours per week. The current hours of operation are Monday through Friday, 7:30 a.m. – 4:30 p.m., excluding federal holidays for contractor personnel. Work hours will be scheduled by the Pharmacy Active Duty Supervisor or Element Chief. Work hours will be scheduled at least 7 days in advance. 4.7. ORIENTATION. The Contractor shall ensure that all contract personnel participate in the MTF orientation procedures for newly assigned personnel to include regulations specific to their professional specialty and clinic and Air Force policy and procedures. 4.8. SECURITY AND PRIVACY REQUIREMENTS All contractor personnel shall be designated as ADP I, II, or III as defined in DOD Regulation 5200.2-R, Appendix K. Personnel assigned this task are required to have a favorable National Agency Check (NAC) for performing in these positions. The Contractor may be required to have access to live data during the performance of this task order. Any information, records, or data the Contractor may have access to may be of a sensitive nature. The Contractor shall not divulge any information about Government files, source selection activities and processes, or any other sensitive information to anyone not authorized access to such information. The contractor shall use patient information for their designated tasks only. This information shall not be used to create databases or any other product not intended for use specifically for their tasks. All patient information related to their tasks shall be destroyed at the conclusion of the tasking. Contractor will adhere to the Privacy Act when providing information. Patient information, no matter how developed, shall be treated as privileged information and shall not be disclosed or revealed in any way for any use outside the MTF without prior written permission by the Chief, Hospital Services. The contractor shall maintain, transmit, retain in strictest confidence, and prevent the unauthorized duplication, use, and disclosure of patient information. The contractor shall provide patient information only to employees, contractors, and subcontractors having a need to know such information in the performance of their duties for this project. 4.9. OTHER. 4.9.1. The Contractor is required to begin full performance of the contract requirements within 30 days after contract award. 4.9.2. The Contractor must take specific action, including Primary Source Verification, to ensure that contract personnel applications have the required prerequisites for application and do not have disqualifying impediments for employment at the MTF. This is a nondelegable responsibility of the Contractor. 5. APPENDICES 5.1. AFI 44-102 SECTION 1N, PHARMACY SERVICES 5.2. DoD REGULATION 5200.2-R, APPENDIX K 5.3. MDGI 44-8 – entirety 5.4.
