Description
The Centers for Disease Control and Prevention (CDC), Native Diabetes Wellness Program (NDWP), Program Development Branch, Division of Diabetes Translation, intends to award, on a sole source basis, a purchase order in the amount of $68,000.00 to the Choctaw Nation of Oklahoma. The Choctaw Nation of Oklahoma will provide support for the coordination of the behavioral health track and transportation services for disabled participants at the HOSW international conference to be held August 6-11, 2006 in Edmonton, Alberta, Canada. The tasks required to complete this action include: (1) coordinating with the international planning committee; (2) administration of support provided for meeting rooms, media and advertising, moderators, recorders and supplies required for a diabetes track at the conference; (3) provision of transportation services for elderly and disabled conference participants. This service and product can only be done to assure a behavioral health track and transportation services for elders and physically disabled participants at the Healing Our Spirits Worldwide (HOSW) international conference. The Choctaw Nation is the designated conference coordinator for the U.S.. The CDC feels that no other provider could provide the product as the Choctaw Nation was designated as the conference coordinator for the HOSW international conference. The period of performance will be from January 20, 2006 to June 30, 2006. Interested parties who feel that they possess the capabilities to perform the services described above, should provide a capabilities statement demonstrating their qualifications, experience, equipment and ability to perform the requirements. The information received in response to this notice will be considered solely for the purpose of determining whether or not to conduct a competitive procurement.
