Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement along with its attachments constitutes the solicitation. This Request for Proposal (RFP) number 06DCL070010, is for the purchase of a Medical Waste Disposal Mail-back Service System. The items needed are 1075 each 2 Gallon Sharps Container; 140 each 5 Gallon Medical Waste Container; 100 each 1 Quart Medical Waste Container; 20 each Locking or Basket Wall Mount with Brackets that fit the 2 Gallon Sharps Container - all for the base year. PLEASE NOTE: the Statement of Work, site listing of all the Occupational Health Centers to be serviced and the estimated quantity per year will be attached to this combined synopsis/solicitation. Solicitation documents, provisions and clauses herein are those in effect through Federal Acquisition Circular 97-18. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items (JAN 2006), and the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items (SEP 2005), applies to this acquisition. Offerors shall submit a fully executed copy of the provision at FAR 52.212-3, Offerors Representations and Certifications--Commercial Items (MAR 2005) with their offer. The clause at FAR 52.212-5, Contract Terms and Conditions Require to Implement Statutes or Executive Orders--Commercial Items (JAN 2006), applies to this acquisition as well as the following clauses cited therein: FAR 52.222-26, Equal Opportunity (E.O. 11246); FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era (29 U.S.C. 793); FAR 52-222-36, Affirmative Action for Workers with Disabilities (29 U.S.C. 793); FAR 52.222-37, Employment Reports on Disabled Veterans of the Vietnam Era (38 U.S.C. 4212); FAR 52.225-3, Buy American Act - Supplies (41.U.S.C.) and FAR 52.232-33, Payment by Electronic Funds Transfer-- Central Contractors Registration (31 U.S.C. 3332). Firms having the ability to provide the service, as stated above, shall provide clear and comprehensive information supporting its claim, and phone number(s); technical literature; GSA schedule, if applicable; and pricing.
