Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and written solicitation will not be issued. Solicitation N61339-06-T-0015 is issued as a request for quote and incorporates the provisions and clauses in effect through Federal Acquisition Circular 2001-1. This acquisition is not a total small business set-aside. The North American Industry Classification System (NAICS) Code for this solicitation is 713940 with a size standard of $6.5 MIL. The following support is required. The Government has a requirement to provide memberships to a Physical Fitness Facility as a part of the PEO STRI Health Awareness Program and Empowerment (SHAPE). The Government organizations requiring access to the facility are: The Program Executive Office for Simulation, Training and Instrumentation (PEO STRI), Naval Air System (NAVAIR) Orlando TSD, and the Air Force Agency for Modeling and Simulation (AFAMS). The memberships will provide facility access to the military members, their dependents, and Government civilians. The quote should include a breakout of the services included in your price (e.g. number of memberships, pool if applicable, aerobics classes, etc). Criteria as follows. Minimum requirements: Total Contract Price: Exercise facilities: Free weights and benches (adequate number and variety) Resistance machines must be adequate to address a full range of upper and lower body strength exercises Aerobic machines including stationary/recumbent bikes, treadmills, stairclimbers, and elliptical trainers Selection of high and low impact type aerobic classes Adequate equipment to insure a minimal wait time for any type of equipment during morning and after work peak usage times Relatively recent equipment (generally < 5 years old) Availability of spotters Access: Operating hours of 0600 to 2200 during weekdays with comparable weekend access Minimum: Military & Dependents have access at no additional cost. It is also desired that Civilians have access at no additional cost, however the Government is willing to consider alternative proposals for Civilians even though it is not the preferred approach Adequate/lighted parking for facility usage Location/proximity to PEO STRI/NAVAIR Hygiene: Exercise areas are clean and sanitary with available cleaners Locker rooms are clean, sanitary, and adequately sized for usage Enhanced Requirements: Exercise facilities: Additional equipment features such as improved ergonomics and computer tracking Additional exercise classes such as yoga, spinning, kickboxing, etc. Health programs such as weight loss, muscle enhancement, etc. Health and nutrition guidance and classes Organized exercise support groups Pool Access Sauna and steam rooms Spa facilities to include Jacuzzi and massage Nutrition and health food store Day care availability New member introduction Access: Expanded operating (and holiday) hours & multiple locations Access for dependents at reduced or no additional cost ALL factors other than price are of equal importance. When all non-price factors are combined, they are more important than price. The Government is requesting that the pricing be divided into two separate Contract Line Items: CLIN 0001 (1 March 2006-28 February 2007) services in accordance with the Statement of Work (SOW). CLIN 0002 Pool Access Only (1 March 2006-28 February 2007). OPTION Year 1 CLIN 1001 (March 1 2007-29 February 2008). CLIN 1002 OPTION Year 1 Pool Access (1 March 2007-29 February 2008). CLIN 2001 OPTION Year 2 (1 March 2008-28 February 2009). CLIN 2002 OPTION Year 2 Pool Access (1 March 2008-28 February 2009). The Government reserves the right to award one or more contracts to provide the services in the SOW. The award decision will be based on the firm providing the best value to the Government. The Government reserves the right to award to other than the lowest priced offeror or other than the highest rated offeror. The Contract Terms and Conditions will be based on Federal Acquisition Regulation (FAR) part 52.212-4. SOW provided below. Physical Fitness Facility Statement of Work (SOW) 1.The following are the minimum basic requirements for the contracting of memberships to a physical fitness facility. a.Provide a physical fitness center available to the entire PEO STRI / AFAMS / NAVAIR Orlando TSD Government workforce, as part of the PEO STRI Health Awareness Program and Empowerment (SHAPE). b. Operating hours will enable club members to use the facilities before, during, and after the normal duty day identified for this purpose as 0600 to 2200 hrs. The facility will be available for use during weekends with daily operating hours both in the AM and PM. The location must provide adjacent and ample free vehicular parking without the necessity for use of special parking permits. c. The facility must be located within the proximity of PEO STRI. d. The physical fitness center must provide a full range of health services to include both a male and female locker room and shower areas. It must be staffed with on-site personnel capable to provide members both orientation and training regarding the use of free weights and nautilus type gym exercise equipment. It should provide an instructed aerobic program consisting of both low and high impact periods of instruction. e. Facility will provide cardiovascular equipment and upper and lower body weight machines for use by the members. f. Staff personnel shall develop a means to monitor and record the usage of the facility by the participating Government workforce. A suggested means would be to require the members to show their Identification Badge that illustrates that they are a PEO STRI / AFAMS / NAVAIR Orlando TSD Government employee, or military dependent. This Statement of Work does not include any Support Service Contractors. Gym staff personnel shall provide the Government a weekly report of membership usage by the participating Government workforce due each Friday for the duration of the contract, by fax to the number (407) 384-3528, Attn: Gary Ashland. g. Desired (not required) to have available day-care area for children of the persons utilizing gym. h. Desired Pool Access 2.
