Description
The purpose of this amendment is to extend the Combined Solicitation to January 5, 2006. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. * * * THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. * * * Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is F1M0W45235A200 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-06. (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 813110 with a $6,000,000.00 size standard. (v) Contractors shall submit a proposal for a base period of 9-months and 4 one-year option periods. Offerors shall include all costs on the attached price schedule, Attachment 1. All responsible sources may submit a proposal, which shall be considered. (vi) The requirement is Catholic Community Coordinator for Charleston AFB Catholic Chapel in accordance with the attached Statement of Work, Attachment 2. The basic contract period is 1 Jan 06 through 30 Sep 06 on Charleston Air Force Base, South Carolina. (See attached statement of work) (vii) Service must begin 1 Jan 06. (viii) The following clauses and provisions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be assessed electronically at this website: http://farsite.hill.af.mil) FAR 52.212-1, Instructions to Offerors-Commercial. (ix); FAR 52.212-2 Evaluation-Commercial Items Evaluation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Technical capability of the item offered to meet the Government requirement. 2. Price- The Government will evaluate offers for award purposes or the total price for the requirement. PAST PERFORMANCE IS SIGNIFICANTLY MORE IMPORTANT THAN PRICE. *If the lowest priced evaluated responsible offer is judged to have an exceptional performance risk rating, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. Offerors shall provide 3-5 references of your most recent projects.} (x) Contractors shall include a completed copy of FAR 52.212-3, Offerors Representation and Certifications Commercial Item or complete electronic annual representations and certifications at http//orca.bpn.gov. (xi) FAR 52.212-4 Contract Terms and Condition—Commercial Items; (xii) FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders—Commercial Item are incorporated by reference, however, for paragraphs (b) and (c) the following clauses apply; FAR 52.222-21 -Prohibition Of Segregated Facilities; FAR 52.222-26 Equal Opportunity; FAR 52.222-35- Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36 Affirmative Action for Workers With Disabilities; FAR 52.222-37 -Employment Reports On Disabled Veterans And Veterans Of The Vietnam Era; FAR 52.232-33 -Payment by Electronic Funds Transfer--Central Contractor Registration; 52.225-1, Buy American Act - Supplies; (xiii) The following additional clauses are applicable to this procurement. FAR 52.219-6, Notice of Total Small Business Set-Aside (if applicable) FAR 52.222-41 Services Contract Act of 1965 and Wage Determination 94-2473 (Rev.31) DFARS 252.204-7004 -Required Central Contractor Registration (CCR); Contractors must be registered in the CCR database prior to award. Offerol parish celebrations, workshops and meetings. k. will research available civilian priests to provide sacramental/pastoral duties for the Catholic community. Will provide priest information to Chapel staff. Chapel staff in turn will obtain priest for identified duties and authorize pay obligation upon completion of priest duties performed. l. will submit to the Administrative Support Chaplain Assistant requests for reserving Chapel facilities on required Chapel facility request forms IAW Chapel Operating Instructions. m. supervises, assists and monitors the entire Religious Education Program by coordinating the Sacramental Preparation programs (Reconciliation, Eucharist and Confirmation), Elementary School, High School Education Program and Adult Education. n. shall attend at least one (1) professional development conference/workshop yearly (Either Air Force or civilian) at the Air Force’s Expense; as long as funds are available. o. shall recommend annual financial requirements for Religious Education (equipment, literature, supplies and related materials) by submitting budget requirements to the Designated Chaplain and to the Catholic Advisory Council no later than 1 March. p. shall be available at the Religious Education facility when Confraternity of Christian Doctrine (CCD) and Rite of Christian Initiation for Adults (RCIA) and other catholic religious education activities when in session, to provide for the administrative needs of students, teachers and parents. q. the duties listed in the SOW are equal to a level of effort of approximately 40-hours per week. These duties may include any meetings, training, office hours, and programming where attendance is necessary. The contractor will have set and published office hours as recommended by the Designated Chaplain and approved by Contracting Officer. r. when unable to perform services, contractor will provide qualified substitute. All subcontractors performing this job must meet all requirements listed in the statement of work. 9. MATERIALS AND EQUIPMENT: The government will provide all necessary equipment, supplies and materials mutually agreed upon for the fulfillment of this contract. Requests for supplies/equipment are given to the Designated Chaplain, who will resource accordingly.
